原发性外耳道癌30例手术治疗及疗效分析

Surgical treatment and efficacy analysis of 30 cases with primary external auditory canal carcinoma

目的分析30例原发性外耳道癌的手术治疗方式及临床疗效。方法回顾分析2011年11月—2022年1月手术治疗的30例外耳道癌患者的临床资料。总结治疗要点,分析并评价术后1~5年肿瘤特异生存率(TSS)及无瘤生存率(TFS),观察术后并发症。结果30例患者中,鳞癌15例、腺癌(腺样囊性癌、耵聍腺癌、汗腺癌)15例。鳞癌组1年、3年、5年TSS分别为92.3%、84.6%、84.6%,TFS分别为92.3%、84.6%、84.6%。腺癌组1年、3年、5年TSS均为100%,TFS分别为92.3%、84.6%、76.9%。术后切口感染2例,一过性后组脑神经麻痹1例,经过治疗后均康复。结论彻底的手术治疗及辅助放化疗治疗外耳道癌效果良好,术后并发症可控。

Objective To analyze the surgical treatment and clinical efficacy of 30 patients with primary external auditory canal carcinoma.Method A retrospective analysis was conducted on the clinical data of 30 patients with primary external auditory canal carcinoma who had undergone surgical treatment from November 2011 to January 2022.The key points of treatment were summarized.The tumor specific survival rate(TSS)and tumor free survival rate(TFS)1 to 5 years after surgery were analyzed and evaluated.And the postoperative complications were observed.Result Among the 30 patients,there were 15 patients of squamous cell carcinoma(SCC)and 15 cases of adenocarcinoma(including adeno cystic carcinoma,cerumen adenocarcinoma and sweat gland carcinoma).The 1-,3-and 5-year TSS of SCC group were 92.3%,84.6%and 84.6%,respectively,and the TFS were 92.3%,84.6%and 84.6%,respectively.The 1-,3-and 5-year TSS of ceruminous carcinoma group were all 100%and the TFS were 92.3%,84.6%and 76.9%,respectively.There were 2 cases of postoperative incision infection and 1 case of transient posterior cerebral nerve paralysis,all of which recovered after treatment.Conclusion Thorough surgical treatment and adjuvant chemoradiotherapy have good effects on the treatment of external auditory canal carcinoma,and postoperative complications are controllable.
