罗哌卡因复合亚甲蓝局部麻醉微创治疗老年股骨转子间骨折围术期的镇痛效果分析

Analysis of the Analgesic Effect of Ropivacaine Combined with Methylene Blue Local Anesthesia in the Perioperative Period of the Treatment of Elderly Intertrochanteric Fractures

目的 探究罗哌卡因复合亚甲蓝局部麻醉微创救治老年股骨转子间骨折围手术时期的镇痛价值。方法 挑选院内2020年1月-2022年1月救治的70位老年股骨转子间骨折病患作为此次研究对象,随机划分为参照组与研究组,每组各35例。参照组予以罗哌卡因相应麻醉操作,研究组则予以罗哌卡因复合甲蓝开展局部麻醉。两组病患均使用股骨近点防旋髓内钉-II(PFNA-II)内固定手术操作,运用VAS评分对病患手术前、手术中与手术后1、2、3、7d开展疼痛状况评价,同时观察病患围手术期并发症。结果 两组病患手术前、手术中VAS评分对比,差异无统计学意义(P<0.05),手术后1、2、3、7d VAS评分研究组显著低于参照组,差异具备统计学意义(P<0.05)。参照组手术后出现3例谵妄,救治后改善。两组以未形成切口并发症为体现,均得以顺利康复。结论 在老年股骨转子间骨折子间骨折病患微创手术救治中采用罗哌卡因复合亚甲蓝局部麻疹是一种效果突出的麻醉措施。

Objective To investigate the analgesic value of ropivacaine complex with methylene blue in the perioperative period of minimally invasive treatment of elderly intertrochanteric fractures under local anesthesia.Methods A total of 70 elderly patients with intertrochanteric femoral fracture treated from January 2020 to January 2022 were randomly divided into a reference group and a research group,with 35 cases in each group.The reference group was given ropivacaine corresponding anesthesia operation,and the study group was given ropivacaine complex nail blue for local anesthesia.In both groups,the femur near-point anti-rotational intramedullary nail-II(pfna-II)internal fixation surgery was performed,and the VAS score was used to evaluate the pain status of patients before,during,and after surgery for 1,2,3 and 7 days,and perioperative complications were observed.Results There was no significant difference in the VAS scores between the two groups before and during surgery(P<0.05),and the 1,2,3 and 7dVAS scores after surgery were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).Three cases of delirium appeared in the reference group after surgery,which improved after treatment.There were no incision complications in either group,and both were discharged smoothly.Conclusion Ropivacaine combined with methylene blue local anesthesia is a scientific and efficient anesthesia strategy in the minimally invasive surgical treatment of elderly patients with intertrochanteric fractures.
