核磁共振中磁敏感加权成像扫描技术在脊髓损伤诊断及神经功能评价中的作用

摘要 目的:探讨核磁共振(MRI)中磁敏感加权成像(SWI)扫描技术在脊髓损伤(SCI)诊断及神经功能评价中的作用.方法:选取2020年7月至2023年10月本院收治82例SCI患者为研究对象.所有患者均行传统MRI、SWI扫描技术检查.比较传统MRI、SWI扫描技术SCI诊断情况,比较不同病情程度SCI患者的SWI扫描参数和美国国立卫生院卒中量表(NIHSS)评分,采用Pearson分析法评估SCI患者SWI扫描参数与NIHSS评分的相关性.结果:SWI扫描技术诊断脊髓出血、脊髓水肿占比、脊髓出血灶阳性检出率高于传统MRI,差异均有统计学意义(P<0.05).SCI重度患者各向异性分数(FA)低于轻中度患者,表观弥散系数(ADC)、λ1、λ2、NIHSS评分均高于轻中度患者,差异均有统计学意义(P<0.05).经Pearson分析,SCI患者FA值与NIHSS评分呈负相关(r=-0.534,P<0.05);ADC、λ1、λ2值与NIHSS评分呈正相关(r=0.650、0.516、0.520,P<0.05).结论:MRI中SWI扫描技术在脊髓损伤诊断及神经功能评价中具有重要的作用,其可提供关于组织微结构的更多信息,帮助医生更准确地评估病情和制定治疗方案. Objective:To explore the role of susceptibility weighted imaging(SWI)scanning technique in magnetic resonance imaging(MRI)in the diagnosis of spinal cord injury(SCI)and evaluation of neurological function.Methods:A total of 82 patients with SCI admited to our hospital from July 2020 to October 2023 were selected as research objects.All of whom underwent conventional MRI and SWI scanning techniques examination.Comparing SCI diagnosis of conventional MRI and SWI scanning techniques.Comparing SWI scanning parameters and National Institute of Health stroke scale(NIHSS)scores of patients with different degrees of SCI.Pearson analysis was used to analyze the correlation between SWI scanning parameters and NIHSS scores of SCI patients.Results:The percentage of spinal cord haemorrhage,spinal cord oedema,and positive detection rate of haemorrhagic foci were higher in SWI scans than those in conventional MRI,and the differences were statistically significant(P<0.05).The fractional anisotropy(FA)was lower in patients with severe SCI than in patients with mild and moderate SCI,and the apparent diffusion coefficient(ADC),X1,X2 values,and NIHSS scores were higher in patients with severe SCI than those in patients with mild and moderate SCI,and the differences were statistically significant(P<0.05).By Pearson's analysis,FA values of SCI patients were negatively correlated with NIHSS scores(r=-0.534,P<0.05);ADC,λ1,andλ2 values were positively correlated with NIHSS scores(r=0.650,0.516,0.520,P<0.05).Conclusion:The SWI scanning technique in MRI plays an important role in the diagnosis of spinal cord injury and the evaluation of neurological function.It can provide more information about the microstructure of tissues,which can help doctors to more accurately assess the disease condition and formulate treatment plans.
