破瘀活血汤加减联合中药穴位贴敷治疗血瘀型腰椎间盘突出症的临床观察

Clinical Observation on Modified Blood Stasis-Removing Blood-activating Decoction Joined with Herbal Acupoint Application in the Treatment of Blood Stasis Type of Lumbar Disc Herniation

目的:探讨破瘀活血汤加减联合中药穴位贴敷治疗血瘀型腰椎间盘突出症(lumbar disc herniation,LDH)患者腰腿痛的疗效。方法:将LDH腰腿痛患者80例按随机数字表法分为对照组与治疗组各40例。对照组进行常规治疗及护理;治疗组在对照组基础上予破瘀活血汤加减联合中药穴位贴敷护理,疗程均为2周。比较两组视觉模拟评分量表(visual analogue scale,VAS)、日本骨科协会(Japanese orthopaedic association scores,JOA)评分、血瘀证症状评分及临床疗效。结果:治疗后两组患者腰部与腿部VAS评分下降,治疗组下降更明显(P<0.01);治疗后两组患者JOA评分升高,治疗组上升更明显(P<0.01);治疗后两组患者血瘀证症状评分下降,治疗组下降更显著(P<0.01);治疗组总有效率为95.0%(38/40),高于对照组的75.0%(30/40)(P<0.05)。结论:破瘀活血汤加减联合中药穴位贴敷护理能有效减轻血瘀型LDH患者腰腿痛,改善腰椎功能和中医证候,效果优于常规治疗及护理。

Objective:To discuss clinical effects of modified blood stasis-removing blood-activating decoction combined with herbal acupoint application in treating back and leg pain in patients with blood stasis type of lumbar disc herniation(LDH).Methods:Eighty patients with back and leg pain of LDH were allocated to the control group and the treatment group in light of random number table method with 40 cases in each group.The control group accepted routine treatment and nursing;and the treatment group was treated with modified blood stasis-removing blood-activating decoction combined with herbal acupoint application on the basis of the control group,the course of the treatment was two weeks.To compare VAS scores,JOA scores,the symptom scores of blood stasis syndrome and clinical effects between both groups.Results:After the treatment,VAS scores of the back and leg were reduced in the two groups,and the decrease of the treatment group was more evident(P<0.01);after the treatment,JOA scores of the two groups were increased,and the increase of the treatment group was more remarkable(P<0.01);after the treatment,symptom scores of blood stasis syndrome were lowered in the two groups,and the downregulation of the treatment group was more noticeable(P<0.01);total effective rate of the treatment group was 95.0%(38/40),higher than 75.0%(30/40)of the control group(P<0.05).Conclusion:Modified blood stasis-removing blood-activating decoction combined with herbal acupoint application could effectively relieve back and leg pain in patients with blood stasis type of LDH,improve lumbar function and TCM syndromes,and its effects superior to these of conventional treatment and care.
