IL-17和IL-1β基因多态性与AECOPD合并肺部感染的关联

Association of IL-17 and IL-1βgene polymorphisms with pulmonary infection in AECOPD patients

目的分析白细胞介素(IL)-17、IL-1β基因多态性与慢性阻塞性肺疾病急性加重期(AECOPD)合并肺部感染的关联。方法选取2018年1月-2023年6月金华市中心医院收治的AECOPD合并肺部感染患者180例为AECOPD合并感染组,另选取同期收治的未合并肺部感染的AECOPD患者180例为AECOPD组,收集患者住院期间临床资料,分析感染患者病原菌分布特征,检测两组患者外周血IL-17和IL-1β表达水平及其基因多态性。结果AECOPD合并感染组患者共分离出201株病原菌,革兰阴性菌占比最高为50.25%;与AECOPD组比较,AECOPD合并感染组患者血清IL-17、IL-1β水平分别为(21.74±4.72)pg/ml、(18.41±3.30)pg/ml,均升高(P<0.05);两组患者IL-17A rs2275913、rs3819024基因型(AA/AG/GG)频率比较均有统计学意义(P<0.05),IL-1βrs1143634基因型(CC/CT/TT)和等位基因(C、T)频率比较均有统计学意义(P<0.05)。结论IL-17 rs2275913、rs3819024基因型和IL-1βrs1143634基因型与AECOPD合并肺部感染风险密切相关。

OBJECTIVE To analyze the association of interleukin(IL)-17 and IL-1βgene polymorphisms with pulmonary infection in the patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).METHODS A total of 180 AECOPD patients complicated with pulmonary infection who were treated in Jinhua Central Hospital from Jan 2018 to Jun 2023 were assigned as the AECOPD-infection group,meanwhile,180 AECOPD patients who were not complicated with pulmonary infection were chosen as the AECOPD group.The clinical data were collected from the enrolled patients during the hospital stay,the distribution of pathogens isolated from the patients was observed,the expression levels of peripheral blood IL-17 and IL-1βwere detected,and the gene polymorphisms were analyzed.RESULTS Totally 201 strains of pathogens were isolated from the patients of the AECOPD-infection group.The proportion of the gram-negative bacteria was the highest(50.25%).The levels of serum IL-17 and IL-1βof the AECOPD-infection group were respectively(21.74±4.72)pg/ml and(18.41±3.30)pg/ml,higher than those of the AECOPD group(P<0.05).There were significant differences in the frequencies of IL-17A rs2275913 and rs3819024 genotypes(AA/AG/GG)between the two groups of patients(P<0.05);there were significant differences in the frequencies of IL-1βrs1143634 genotypes(CC/CT/TT)and alleles(C,T)between the two groups of patients(P<0.05).CONCLUSION The IL-17 rs2275913 and rs3819024 genotypes and IL-1βrs1143634 genotypes are closely associated with the pulmonary infection in the AECOPD patients.
