消化性溃疡患者血清CGRP、PINP、α-DF水平及其与不同基因型Hp感染的关系

Serum CGRP,PINP andα-DF of patients with peptic ulcer and association with different genotypes of Hp infections

目的探究消化性溃疡(PU)患者血清降钙素基因相关肽(CGRP)、Ⅰ型原胶原N端前肽(PINP)、α-防御素(α-DF)水平及其与不同基因型幽门螺杆菌(Hp)感染的关系。方法选取2019年6月-2023年6月磐安县人民医院收治的155例PU患者为溃疡组,^(14)C尿素呼气试验检测患者Hp感染情况,抗体法检测Hp基因型,另选同期体检健康者161名作为健康对照组,检测两组血清CGRP、PINP、α-DF水平,受试者工作特征(ROC)曲线分析血清CGRP、PINP、α-DF对消化性溃疡患者Hp感染的诊断价值。结果溃疡组血清CGRP、α-DF水平均低于健康对照组(P<0.05),血清PINP水平高于健康对照组(P<0.05);155例PU患者根据是否感染Hp分为Hp阳性组87例和Hp阴性组68例,与Hp阴性组相比,Hp阳性组血清CGRP、α-DF水平降低,血清PINP水平升高(P<0.05);87例Hp阳性患者根据Hp基因型不同分为Hp阳性Ⅰ型组46例和Hp阳性Ⅱ型组41例,Hp阳性Ⅰ型组血清CGRP、α-DF水平低于Hp阳性Ⅱ型组(P<0.05),血清PINP水平高于Hp阳性Ⅱ型组(P<0.05);血清CGRP、PINP、α-DF联合检测诊断PU患者Hp感染的曲线下面积(AUC)值高于其单一检测(P<0.05),且联合检测的敏感度、特异度为83.91%、88.24%。结论PU患者发生Hp感染及不同基因型Hp感染的血清CGRP、α-DF、PINP水平表达异常,且三者联合检测可提高PU患者Hp感染的诊断价值。

OBJECTIVE To explore the levels of serum calcitonin gene-related peptide(CGRP),procollagen I N-terminal propeptide(PINP)andα-defensins(α-DF)of the patients with peptic ulcer(PU)and analyze the association with different genotypes of Helicobacter pylori(Hp)infections.METHODS Totally 155 patients with PU who were treated in Panan County People′s Hospital from Jun 2019 to Jun 2023 were assigned as the ulcer group.The Hp infection was tested for the patients by^(14)C urea breath test,the genotype of Hp were detected by antibody method.Meanwhile,161 healthy people who received physical examination were chosen as the healthy control group.The levels of serum CGRP,PINP andα-DF were detected.The values of serum CGRP,PINP andα-DF in diagnosis of Hp infection in the PU patients were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The levels of serum CGRP andα-DF of the study group were lower than those of the control group(P<0.05),while the serum PINP level of the study group was higher than that of the control group(P<0.05).The 155 PU patients were divided into the Hp-positive group with 87 cases and the Hp-negative group with 68 cases according to the status of Hp infection.The levels of serum CGRP andα-DF of the Hp-positive group were lower than those of the Hp-negative group,while the serum PINP level of the Hp-positive group was higher than that of the Hp-negative group(P<0.05).The 87 Hp-positive patients were divided into the Hp-positive typeⅠgroup with 46 cases and the Hp-positive typeⅡgroup with 41 cases according to the genotypes of Hp.The levels of serum CGRP andα-DF of the Hp-positive typeⅠgroup were lower than those of the Hp-positive typeⅡgroup(P<0.05),while the serum PINP level of the Hp-positive typeⅠgroup was higher than that of the Hp-positive typeⅡgroup(P<0.05).The area under curve(AUC)of the joint detection of serum CGRP,PINP andα-DF was higher than that of the single detection in diagnosis of the Hp infection in the PU patients(P<0.05),the sensitivity of the joint detection was 83.91%,with the specificity 88.24%.CONCLUSION The PU patients with Hp infection and the patients with different genotypes of Hp infections show abnormal expressions of serum CGRP,α-DF and PINP.The joint detection of the three indexes can raise the diagnostic value of Hp infection in the PU patients.
