摘要
目的研究上尿路结石腔道手术后泌尿系统感染Toll样受体(TLRs)介导的炎症信号通路相关因子水平变化。方法随机选择2021年8月-2023年8月在金华市中心医院就诊的上尿路结石并行腔道取石手术术后发生泌尿系统感染患者30例为感染组,30例未发生感染患者为对照组,收集患者临床资料,分析炎性因子白细胞计数(WBC)、C-反应蛋白(CRP)、降钙素原(PCT)、白细胞介素(IL)-17、IL-10、单核细胞趋化蛋白(MCP)-1和转化生长因子-β(TGF-β)水平变化及TLRs信号通路mRNA、蛋白相对表达水平。结果与对照组比较,感染组患者WBC、CRP、PCT、IL-17、MCP-1水平升高(P<0.05),IL-10、TGF-β表达水平降低(P<0.05);TLR4、MyD88、NF-κB mRNA转录水平及蛋白相对表达水平升高(P<0.05);Pearson相关分析结果显示,TLRs介导的炎症信号通路相关因子表达与WBC、CRP、PCT均呈正相关(P<0.05)。结论上尿路结石腔道手术后泌尿系统感染患者TLRs介导的炎症信号通路相关因子表达水平升高,可为术后感染临床预防提供支持。
OBJECTIVE To observe the changes of Toll-like receptors(TLRs)-mediated inflammatory signaling pathway-related factors in the upper urinary calculi(UUC)patients with urinary system infection after luminal lithotomy.METHODS A total of 30 patients with UUC who underwent luminal lithotomy in Jinhua Central Hospital from Aug 2021 to Aug 2023 were assigned as the infection group,and 30 patients who did not have infection were chosen as the control group.The clinical data were collected from the patients.The levels of inflammatory factors white blood cell(WBC),C-reactive protein(CRP),procalcitonin(PCT),interleukin(IL)-17,IL-10,monocyte chemoattractant protein(MCP)and transforming growth factor-β(TGF-β)as well as the relative expression levels of TLRs signaling pathway mRNA and proteins were observed.RESULTS The levels of WBC,CRP,PCT,IL-17 and MCP-1 of the infection group were higher than those of the control group(P<0.05),while the expression levels of IL-10 and TGF-βof the infection group were lower than those of the control group(P<0.05).The transcription levels of TLR4,MyD88 and NF-κB mRNA as well as the relative expression levels of proteins of the infection group were higher than those of the control group(P<0.05).Pearson correlation analysis showed that the expressions of the TLRs-mediated inflammatory signaling pathway-related factors were positively correlated with WBC,CRP and PCT(P<0.05).CONCLUSION The UUC patients who have urinary system infection after luminal lithotomy show the rise of expression levels of TLRs-medicated inflammatory signaling pathway-related factors,which may support the clinical prevention of postoperative infection.
