外固定支架复位联合克氏针治疗对桡骨远端骨折愈合时间、影像学参数的影响

The effect of external fixator reduction combined with Kirschner wire treatment on fracture healing time and imaging parameters in patients with distal radius fractures

目的探讨外固定支架复位联合克氏针治疗对桡骨远端骨折患者骨折愈合时间、影像学参数的影响。方法选取2020年1月-2023年2月淄博市博山区中医院骨三科收治的86例桡骨远端骨折患者为研究对象,按照随机数表法分为两组。对照组(n=43例)给予手法复位小夹板固定治疗,观察组(n=43例)实施外固定支架复位联合克氏针治疗。对比两组相关症状改善时间、腕关节活动情况、桡骨短缩长度和骨密度、影像学指标、并发症。结果观察组骨折愈合时间、疼痛改善时间、手背消肿时间均短于对照组,差异有统计学意义(P<0.05);术后3个月,观察组腕关节旋前活动度、旋后活动度、背伸活动度、掌屈活动度、骨密度、掌倾角、尺偏角均高于对照组,观察组桡骨短缩长度短于对照组,差异有统计学意义(P<0.05);观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论在桡骨远端骨折患者中应用外固定支架复位联合克氏针治疗效果显著,能够有效缩短骨折愈合时间,提高腕关节活动度,降低机体内疼痛程度,减少并发症的发生。

Objective To explore the effect of external fixation and reduction combined with Kirschner wire treatment on fracture healing time and imaging parameters in patients with distal radius fractures.Methods From January 2020 to February 2023,86 patients with distal radius fracture who were admitted to the Third Department of Orthopaedics of Boshan District Hospital of Traditional Chinese Medicine were selected as the research objects,and were divided into two groups according to the random number table.The control group(n=43 cases)received manual reduction and small splint fixation treatment,while the observation group(n=43 cases)received external fixation bracket reduction combined with Kirschner wire treatment.Compare the improvement time of related symptoms,wrist joint activity,radius shortening length and bone density,imaging indicators,and complications between the two groups.Results The fracture healing time,pain improvement time,and hand back swelling reduction time in the observation group were shorter than those in the control group,with statistical differences(P<0.05);After 3 months of surgery,the wrist joint pronation activity,supination activity,back extension activity,palm flexion activity,bone density,palm inclination angle,and ulnar deviation angle in the observation group were higher than those in the control group.The radius shortening length in the observation group was shorter than that in the control group,with a statistical difference(P<0.05);The total incidence of complications in the observation group was lower than that in the control group,with a statistical difference(P<0.05).Conclusion The application of external fixation and reduction combined with Kirschner wire in the treatment of distal radius fractures has a significant effect,which can effectively shorten the fracture healing time,improve wrist joint mobility,reduce internal pain,and reduce the occurrence of complications.
