探讨经皮椎体后凸成形术(PKP)治疗无脊髓压迫症状创伤后脊柱塌陷的疗效及安全性

目的探究与分析经皮椎体后凸成形术(PKP)治疗无脊髓压迫症状创伤后脊柱塌陷的疗效及安全性.方法回顾性分析江苏省如皋市中医院自2021年5月-2023年3月收治的无脊髓压迫症状创伤后脊柱塌陷患者90例的临床资料,按照手术操作分为PKP组(n=49)与经皮穿刺椎体成形术(PVP)组(n=41),对比两组围术期指标、手术前后椎体高度改善情况以及Cobb角,对比两组并发症发生情况.结果PKP组与PVP组比较,手术时间较长、骨水泥注入量较多,椎体前壁较高、椎体中线较高;PKP组治疗后与PVP组治疗后比较,ODI评分较低、Cobb角度较低,差异有统计学意义(P<0.05).两组术后并发症发生率比较,差异无统计学意义(P>0.05).结论PKP治疗无脊髓压迫症状创伤后脊柱塌陷的围术期指标表现较好,患者椎体高度得到了明显的改善,术后并发症发生率风险较低,安全性有保证.

Objective To investigate and analyze the efficacy and safety of percutaneous kyphoplasty(PKP)in the treatment of post-traumatic spinal collapse without symptoms of spinal cord compression.Methods Clinical data of 90 cases of post-traumatic spinal collapse without symptoms of spinal cord compression admitted to our hospital from May 2021 to March 2023 were retrospectively analyzed.They were divided into PKP group(n=49)and percutaneous vertebroplasty(PVP)group(n=41)according to surgical procedures.Perioperative indexes,improvement of vertebral height before and after surgery,and Cobb Angle of the two groups were compared.Complications were compared between the two groups.Results Compared with PVP group,PKP group had longer operation time,more bone cement injection,higher anterior vertebral wall and higher midline;there were significant differences in ODI score and Cobb Angle between PKP group and PVP group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion PKP has good perioperative performance in the treatment of post-traumatic spinal collapse without spinal cord compression symptoms,and the vertebral body height of patients has been significantly improved.The risk of postoperative complications is low,and the safety is guaranteed.
