摘要
目的综合分析2017-2020年通州区孕妇单增李斯特菌感染案例及市售食品单增李斯特菌感染情况,提出预防和控制措施.方法对实验室确诊的单增李斯特菌感染孕妇开展流行病学调查,采集孕妇及新生儿生物样本及家庭环境样本进行实验室检测,判断感染来源.结果6名孕妇临床表现为发热、不规律下腹痛等,5人在围生期发现,1人在非围生期发现,1名孕妇出现胎死宫内的不良结局;感染孕妇及新生儿生物样本培养单增李斯特菌检测结果均为阳性;流行病学调查发现感染孕妇均有食用熟肉制品等高危饮食行为,家庭卫生习惯中未做到生熟分开及冰箱定时清洁.2017-2020年通州区市售食品单增李斯特菌监测中发现,熟肉制品中单增李斯特菌检出率最高,为8.7%.结论孕妇高危饮食行为和较差的家庭卫生习惯可能是单增李斯特菌感染来源;应加强孕妇食品安全健康教育,改善不良家庭卫生习惯,加强医院的早期识别,避免新生儿不良结局出现.
Objective To investigate and analyze the cases of Listeria monocytogenes infection in pregnant women and to understand the contamination status of Listeria monocytogenes in foods sold in Tongzhou District of Beiing during 2017-2020,and to propose prevention and control measures.Methods An epidemiological investigation of laboratory-confirmed case of Listeria monocytogenes infection in pregnant women was carried out.The biological samples from pregnant women and newborns and family environmental samples was collected for Listeria monocytogenes test,and the source of infection was analyzed.Results The clinical manifestations of 6 pregnant women were fever,irregular lower abdominal pain,etc.,5 were found in perinatal period,1 were found in non-perinatal period,and 1 pregnant woman had an adverse outcome of fetal death in the uterus.The detection results of Listeria monocytogenes cultured in biological samples of infected pregnant women and newborns were positive.Epidemiological investigation found that infected pregnant women had high-risk dietary behaviors such as eating cooked meat products,and did not separate raw and cooked foods and regularly clean refrigerators in family health habits.It was found that the detection rate of Listeria monocytogenes in cooked meat products was the highest,8.7%during 2017-2020.Conclusions The high risk dietary behaviors and poor household hygiene habits may be the main pathogenic factor of listeriosis in this case of pregnant woman.Strengthening the health education on food safety in pregnant women and improving bad family health habits.The early identification in hospitals could help prevent and treat the disease,reduce adverse neonatal outcomes.
