周振理教授中西医结合复方大承气汤治疗粘连性肠梗阻的临床研究

Professor Zhou Zhenli's clinical study of Dachengqi Decoction combined Traditional Chinese and Western Medicine in the treatment of adhesive intestinal obstruction

目的研究在为粘连性肠梗阻(Adhesive intestinal obstruction,AIO)患者实施治疗的过程中周振理教授中西医结合复方大承气汤的临床应用效果.方法此次试验研究将2022年1月-12月期间接受常规中西医治疗的AIO患者18例作为对照组,将2023年1月-12月期间接受周振理教授中西医结合复方大承气汤治疗的AIO患者18例作为观察组,比较两种治疗方案的应用效果.结果观察组治疗有效率为94.4%,对照组治疗有效率为77.8%,观察组患者更具优势,差异有统计学意义,P<0.05.观察组患者首次通气时间(3.09±0.72)d、首次进食时间(4.87±0.91)d、首次排便时间(4.21±0.53)d、肠鸣音恢复时间(2.08±0.35)d、腹部症状缓解时间(3.17±0.62)d、住院时间(9.28±1.06)d均相对较短,显著差别于对照组患者,P<0.05.观察组患者治疗后排便排气积分(0.67±0.21)分、恶心呕吐积分(0.53±0.17)分、腹部胀痛积分(0.97±0.36)分、口苦口干积分(0.87±0.26)分,相比对照组症状积分更低,存在显著统计学差异,P<0.05.而观察组患者治疗后C反应蛋白为(17.23±2.47)mg/L、血沉为(12.47±2.03)mm/h、白细胞计数为(8.52±1.40)×109/L、中性粒细胞百分比为(74.63±3.25)%,以上临床指标水平相对较低,显著差别于对照组患者,P<0.05.结论AIO患者实施治疗时周振理教授中西医结合复方大承气汤的应用效果相对较为理想,不仅可以获得相对较为显著的治疗效果,同时也改善了患者的临床症状与相关指标水平,缩短了患者首次通气、首次进食、首次排便、肠鸣音恢复、腹部症状缓解以及住院时间.

Objective To study the clinical effects of Prof.Zhou Zhenli's Traditional Chinese Medicine combined with compound Dachengqi Decoction in the treatment of Adhesive intestinal obstruction(AIO).Methods In this study,18 AIO patients who received conventional Traditional Chinese and Western Medicine treatment in hospital from January to December 2022 were selected as the control group,and 18 AIO patients who received Traditional Chinese and Western Medicine combination Dachengqi Decoction in hospital from January to December 2023 were selected as the observation group.The clinical effects of different treatment schemes were compared and analyzed.Results The effective rate of observation group was 94.4%.The effective rate of the control group was 77.8%,and the observation group was more advantageous,with statistical significance(P<0.05).In the observation group,the time of first ventilation was(3.09±0.72)d,the time of first feeding was(4.87±0.91)d,the time of first defecation was(4.21±0.53)d,the time of intestinal sound recovery was(2.08±0.35)d,the time of abdominal symptom relief was(3.17±0.62)d,and the time of hospitalization was(9.28±1.06)d.Compared with control group,there was significant statistical significance,P<0.05.After treatment,the score of defecation and exhaust in observation group was(0.67±0.21)points,the score of nausea and vomiting was(0.53±0.17)points,the score of abdominal distension and pain was(0.97±0.36)points,and the score of bitter mouth and dry mouth was(0.87±0.26)points,which was relatively lower than that of control group,and there was significant statistical significance,P<0.05.After treatment,the C-reactive protein in observation group was(17.23±2.47)mg/L,erythrocyte level was(12.47±2.03)mm/h,white blood cell count was(8.52±1.40)×109/L,and neutrophil percentage was(74.63±3.25)%.The above clinical indexes were relatively low,significantly different from those in control group,P<0.05.Conclusion In the treatment of AIO patients,the application effect of Professor Zhou Zhenli's combined Traditional Chinese and Western Medicine compound Dachengqi Decoction is relatively ideal,which can not only obtain relatively significant therapeutic effect,but also improve the clinical symptoms and related indicators of patients,and shorten the clinical time index of patients,which is worthy of clinical promotion.
