利疸消鼓汤联合穴位贴敷治疗肝硬化腹腔积液的临床研究

Clinical study of liyeng xiaogu decoction combined with acupoint application on ascites of liver cirrhosis

目的:探讨利疸消鼓汤联合穴位贴敷治疗肝硬化腹腔积液的临床效果。方法:采用随机数字表法将2022年8月-2023年8月收治的60例湿热夹瘀型肝硬化腹腔积液患者分为观察组和对照组,每组30例。对照组采用西医基础治疗,观察组采用西医基础治疗+利疸消鼓汤联合穴位贴敷治疗,治疗周期均为2个疗程,共28 d。比较两组临床疗效、超声腹腔积液积分、中医证候积分、体质量、腹围、24 h尿量、凝血酶原时间(PT)和肝功能[丙氨酸氨基转移酶(ALT)、白蛋白(ALB)、总胆红素(TBIL)、天门冬氨酸氨基转氨酶(AST)]以及Child-Pugh分级。结果:观察组治疗总有效率高于对照组,治疗后超声腹腔积液积分、中医证候积分、体质量、腹围、24 h尿量及凝血酶原时间(PT),ALT,TBIL,AST水平低于对照组,ALB水平高于对照组,Child-Pugh分级优于对照组(P<0.05)。结论:应用利疸消鼓汤联合穴位贴敷治疗湿热夹瘀型肝硬化腹腔积液患者效果确切,利于改善腹腔积液症状、肝功能和凝血功能。

Objective:To investigate the clinical effect of Li jaundice and eliminating drum soup combined with acupoint application on ascites of liver cirrhosis abdominal effusion.Methods:All 60 patients with ascites with damp-heat and blood-stasis cirrhosis admitted to our hospital from August 2022 to August 2023 were divided into two groups by random number table method.Thirty patients receiving basic Western medicine treatment were set as the control group,and 30 patients receiving basic Western medicine treatment plus Li jaundice and eliminating drum soup combined with acupoint application were set as the observation group.The treatment cycle of both groups was 2 courses,a total of 28 days.The clinical efficacy,ultrasonic ascites score,traditional chinese medicine syndrome score,body weight,abdominal circumference,24h urine volume,prothrombin time(PT),liver function[alanine aminotransferase(ALT),albumin(ALB),total bilirubin(TBIL),aspartate aminotransferase(AST)]and Child-Pugh grades of the two groups were compared.Results:The total effective rate of the observation group was higher than that of the control group,the ultrasonic ascites score,traditional chinese medicine syndrome score,abdominal circumference,24h urine volume,PT,ALT,TBIL and AST levels after treatment were lower than those of the control group,the ALB level was higher than that of the control group,and the Child-Pugh grade was better than that of the control group(P<0.05).Conclusion:The application of Li jaundice and eliminating drum soup combined with acupoint application in the treatment of ascites patients with damp-heat and blood-stasis cirrhosis is effective and beneficial to improving ascites symptoms,liver function and coagulation function.
