吊床体位在高胆红素血症新生儿蓝光照射治疗中的应用研究

摘要 目的探讨吊床体位在高胆红素血症新生儿蓝光照射治疗中的应用效果。方法采用便利抽样法,选取2023年1-12月福建医科大学附属第二医院新生儿重症监护室收治的120例高胆红素血症患儿作为研究对象,采用随机数字表法将其分为对照组和观察组,各60例。对照组采用蓝光照射治疗常规体位,观察组在蓝光照射治疗过程中采取吊床体位干预,比较两组光疗总时间、黄疸消退时间及疼痛与不适评分。结果观察组光疗总时间和黄疸消退时间均短于对照组(t=3.600,P<0.001;t=6.473,P<0.001);两组疼痛与不适评分在时间效应、组间效应及交互效应方面比较,差异均有统计学意义(F时间=818.404,P<0.001;F组间=91.648,P<0.001;F交互=48.713,P<0.001)。结论将吊床体位应用于高胆红素血症新生儿蓝光照射治疗中,可提高蓝光照射治疗效果,缩短光疗时间,提高患儿舒适度。 Objective To explore the effect of applying hammock position in the treatment of neonatal hyperbilirubinemia using blue light irradiation.Methods A total of 120 neonatal with hyperbilirubinemia admitted to neonatal intensive care unit in 2023 were selected and divided into a control group and an observation group,each of 60,according to a random number table.Both groups received routine blue light irradiation,while the observation group was given hammock position intervention.The duration of blue light irradiation treatment,jaundice resolution time and neonatal pain and discomfort scores were compared between the two groups.Results The duration of blue light exposure and jaundice resolution in the observation group were significantly shorter than that of the control group(t=3.600,P<0.001;t=6.473,P<0.001).Moreover,significant differences were found between the two groups in terms of time effect,intergroup effect and interaction effect(F time=818.404,P<0.001;F intergroups=91.648,P<0.001;F interaction=48.713,P<0.001).Conclusion The hammock position can improve the effect of blue light irradiation,shorten the time of light therapy and relieve the discomfort of children with hyperbilirubinemia.
