摘要
目的评价迈瑞BC-6800 Plus全自动血液分析仪(简称BC-6800 Plus分析仪)8倍倍增模式——利用光学法原理+半导体激光+RNA核酸荧光染色法(CDR)通道模式检测电阻法血小板(PLT)假性异常的性能。方法选择2021年2月至2023年5月BC-6800 Plus分析仪电阻法PLT假性异常标本89例,分别采用电阻法、CDR通道模式及手工计数法进行PLT检测,比较3种方法检测结果的差异性。同时采用CDR通道模式连续检测PLT 10次,评价其重复性,并以手工计数法为参照,评价其准确性和一致性。结果BC-6800 Plus分析仪CDR通道模式检测电阻法PLT假性升高标本结果[(231.81±57.75)×10^(9)/L]明显低于电阻法[(586.81±124.99)×10^(9)/L],而假性减少标本结果[(189.37±51.06)×10^(9)/L]明显高于电阻法[(43.52±20.10)×10^(9)/L],差异有统计学意义(P<0.05);但与手工计数法[(218.95±51.26)×10^(9)/L和(206.37±56.07)×10^(9)/L]差异无统计学意义(P>0.05);BC-6800 Plus分析仪CDR通道模式检测电阻法PLT假性升高和减少标本CV值分别为4.33%和5.20%,均<10.00%;以手工计数结果为靶值,BC-6800 Plus分析仪CDR通道模式检测电阻法PLT假性升高和减少标本Bias分别为7.56%和-5.28%,均<10.00%;经Pearman相关分析,BC-6800 Plus分析仪CDR通道模式检测电阻法PLT假性异常标本与手工计数法结果呈明显正相关(r_(升高标本)=0.9613,r_(减少标本)=0.9401,P<0.05)。结论BC-6800 Plus分析仪CDR通道模式检测电阻法PLT假性异常标本中PLT与手工计数法结果具有很好的一致性,且重复性好、正确度高,能解决红细胞(RBC)碎片、小RBC、大PLT及PLT聚集对PLT检测的干扰,值得临床推广应用。
Objective To evaluate the performance of 8-fold multiplication mode of Mindray BC-6800 Plus automatic blood analyzer(BC-6800 Plus analyzer)--using optical method principle+semiconductor laser+RNA nucleic acid fluorescence staining method(CDR)in detecting of pseudo-abnormality in platelet(PLT)by resistance method.Methods From February 2021 to May 2023,a total of 89 cases of PLT pseudo-abnormal specimens were detected by BC-6800 Plus analyzer.The resistance method,CDR channel mode and manual counting method were used to detect PLT,and the differences of results from the three methods were compared.At the same time,CDR channel mode was used to continuously detect PLT for 10 times and evaluate its repeatability,and manual counting method was used as reference to evaluate its accuracy and consistency.Results The outcome of pseudo-elevated specimens of CDR channel mode of BC-6800 Plus analyzer in detection of PLT by resistance method[(231.81±57.75)×10^(9)/L]were significantly lower than those by resistance method[(586.81±124.99)×10^(9)/L],while the outcome of pseudo-abnormal reduction specimens of[(189.37±51.06)×10^(9)/L]were significantly higher than those of resistance method[(43.52±20.10)×10^(9)/L],and the differences were statistically significant(P<0.05).However,the outcomes had no significant differences when compared with that of manual counting method(P>0.05).The specimen CV values pseudo-elevated specimens and pseudo-abnormal reduction specimens of detection PLT increase and decrease by resistance method CDR channel mode of BC-6800 Plus analyzer by 4.33%and 5.20%,respectively,both<10.00%.The results of manual counting were used as the target value,the Bias of pseudo-elevated specimens and pseudo-abnormal reduction specimens of PLT detected by BC-6800 Plus analyzer CDR channel mode increase and decrease by 7.56%and-5.28%,respectively,both<10.00%.By Pearman correlation analysis,the pseudo-abnormal specimens of BC-6800 Plus analyzer CDR channel mode detection resistance method PLT and manual counting method results showed a significant positive correlation(relevated specimen=0.9613,rreduced specimens=0.9401,P<0.05).Conclusion It is demonstrated that BC-6800 Plus analyzer CDR channel pattern detection shows good consistency in pseudo abnormal PLT specimens detected by resistance method and that by manual counting methods.It has good repeatability and high accuracy,and can resist detection interference from red blood cell(RBC)debris,small RBC,large PLT and PLT aggregation,which is worthy of clinical application.
