柔肝散结汤加减联合富马酸替诺福韦二吡呋酯片治疗慢性乙型肝炎的效果

Effect of Rougan Sanjie Decoction Combined with Tinofovir Dipifuryl Fumarate Tablet on Chronic Hepatitis B

目的:观察在慢性乙型肝炎治疗上选择柔肝散结汤加减联合富马酸替诺福韦二吡呋酯片的应用效果。方法:选取2021年1月—2023年6月宁化县总医院收治的178例慢性乙型肝炎患者作为研究对象,按照随机数表法分为对照组和观察组,每组各89例。对照组采用柔肝散结汤加减法,观察组在对照组基础上联合富马酸替诺福韦二吡呋酯片。观察两组临床效果、谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBil)、乙型肝炎病毒DNA(HBV-DNA)、肝弹性硬度、HBeAg、HBV-DNA 转阴率、HBeAg转阴率、不良反应发生情况及生活质量。结果:观察组较对照组的疗效更高,差异有统计学意义(P<0.05);治疗后AST、ALT、TBil、HBV-DNA 、肝弹性硬度和HBeAg比对照组低,差异有统计学意义(P<0.05);观察组的HBV-DNA转阴率和HBeAg转阴率较对照组更高,差异有统计学意义(P<0.05);两组不良反应,差异无统计学意义(P>0.05);治疗前,两组生活质量比较,差异无统计学意义(P>0.05);治疗后,观察组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论:在慢性乙型肝炎治疗上选择柔肝散结汤加减联合富马酸替诺福韦二吡呋酯片具有良好疗效,其可有效改善患者各临床指标,提升HBV-DNA 及HBeAg 转阴率,提高生存质量,改善预后。

Objective:To observe the application effect of the combination of modified Rougan Sanjie Decoction and Tenofovir Fumarate Dipyrfuroxime Tablet in the treatment of chronic hepatitis B.Method:178 patients with chronic hepatitis B admitted to Ninghua County General Hospital from January 2021 to June 2023 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 89 cases in each group.Control group was given Ruogan Sanjie Decoction plus subtraction and observation group was given Tenofovir dipifurate fumarate tablet.Clinical effects,aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBil),hepatitis B virus DNA(HBV-DNA),liver elastic hardness and HBeAg,HBV-DNA negative conversion rate and HBeAg negative conversion rate,adverse reactions and quality of life were observed.Result:The curative effect of observation group was higher than that of control group,the difference was statistically significant(P<0.05).After treatment,AST,ALT,TBil,HBV-DNA,liver elastic hardness and HBeAg in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).HBV-DNA negative conversion rate and HBeAg negative conversion rate in observation group were higher those in control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group and the observation group(P>0.05).Before treatment,there was no significant difference in quality of life between the two groups(P>0.05).After treatment,the quality of life score of the observation group was higher than that of control group,and the difference was statistically significant(P<0.05).Conclusion:Ruogan Sanjie Decoction combined with Tenofovir dipifurate fumarate tablet has good curative effect in the treatment of chronic hepatitis B,which can effectively improve clinical indicators,increase the HBV-DNA and HBeAg negative conversion rate,improve the quality of life and improve the prognosis of patients.
