收集病灶冲洗液行液基细胞学检查替代刷检在肺癌诊断中的价值

摘要 目的 探讨收集病灶冲洗液行液基细胞学检查替代刷检在肺癌诊断中的价值。方法 120例疑诊肺癌患者为研究对象,其中经黏膜组织活检确诊肺癌100例。所有患者均行刷检涂片细胞学检查、收集病灶冲洗液行液基细胞学检查。比较两种取材方法操作时间及取材后病灶出血情况;两种取材方法对经黏膜组织活检确诊患者的阳性检出情况。结果 收集病灶冲洗液取材操作时间(3.07±0.27)min短于刷检取材的(4.27±0.20)min,差异具有统计学意义(P<0.05)。收集病灶冲洗液取材后病灶出血率0.83%(1/120)低于刷检取材的5.83%(7/120),差异具有统计学意义(P<0.05)。经黏膜组织活检确诊患者100例,收集病灶冲洗液取材方法对经黏膜组织活检确诊患者的阳性检出率为83.00%,高于刷检取材方法的59.00%,差异具有统计学意义(P<0.05)。结论 与刷检涂片细胞学检查比较,收集病灶冲洗液行液基细胞学检查在肺癌诊断中的价值高,能缩短取材时间,降低取材后病灶出血率,提高阳性检出率,及时且较准确地检出患者病情。 Objective To explore the value of collecting lesion irrigation fluid for liquid-based cytology instead of brush biopsy in the diagnosis of lung cancer.Methods A total of 120 patients with suspected lung cancer were selected as the study subjects,and 100 patients were diagnosed with lung cancer by mucosal tissue biopsy.All patients underwent brush smear cytology and collected lesion irrigation fluid for liquid-based cytology.Comparison was made on operation time and lesion bleeding after sampling between the two methods,as well as the positivity detection of the two sampling methods in patients confirmed by mucosal tissue biopsy.Results The time taken to collect lesion irrigation liquid was(3.07±0.27)min,which was shorter than(4.27±0.20)min of brush biopsy,and the difference was statistically significant(P<0.05).The bleeding rate of the lesion was 0.83%(1/120)after the collection of lesion irrigation liquid,which was lower than 5.83%(7/120)of brush sampling,and the difference was statistically significant(P<0.05).In 100 cases of patients diagnosed by mucosal tissue biopsy,the positive detection rate of collecting lesion irrigation fluid was 83.00%,which was higher than 59.00%of brushing sampling,and the difference was statistically significant(P<0.05).Conclusion Collecting lesion irrigation fluid for liquid-based cytology instead of brush biopsy has high value in the diagnosis of lung cancer,which can shorten the time taken to collect the sample,reduce the bleeding rate of the lesion after the collection,improve the positive rate of diagnosis,and detect the patient's condition in a timely and accurate manner.
