主动运动干预对膝关节骨关节炎患者疼痛、心理状态及功能、步态恢复的影响

Effects of active exercise intervention on pain,psychological state,function and gait recovery in patients with knee osteoarthritis

目的:研究主动运动干预对膝关节骨关节炎(KOA)患者疼痛、心理状态及功能、步态恢复的影响。方法:纳入2021年3月至2022年10月期间厦门大学附属成功医院收治的120例KOA患者以随机数字表法分为被动组与主动组各60例,被动组实施被动运动干预,主动组在被动组基础上实施主动运动干预,两组患者均干预4周,比较两组患者干预前后疼痛与僵硬症状[西安大略和麦克马斯特大学骨关节炎指数(WOMAC)]、心理状态[医院焦虑抑郁量表(HADS)]、膝关节功能(Lysholm膝关节评分)、步态(步长、步速、支撑相占比)。结果:干预后,两组患者WOMAC、HADS评分及支撑相占比较干预前显著降低,且主动组低于被动组(P<0.05);两组患者Lysholm膝关节评分、步长、步速较干预前显著上升,且主动组高于被动组(P<0.05)。结论:主动运动干预KOA患者可有效缓解其疼痛感,促进患者膝关节功能及步态恢复。

Objective:To study the effects of active exercise intervention on pain,psychological state,function and gait recovery in patients with knee osteoarthritis(KOA).Methods:A total of 120 KOA patients admitted to Chenggong Hospital Affiliated to Xiamen University from March 2021 to October 2022 were divided into passive group and active group by random number table method,with 60 cases in each group.The passive group was given passive exercise intervention,while the active group was given active exercise intervention on the basis of the passive group.Both groups of patients were intervened for 4 weeks.The pain and stiffness symptoms[Western Ontario and McMaster University Osteoarthritis Index(WOMAC)],psychological state[Hospital Anxiety and Depression Scale(HADS)],knee function[Lysholm knee score]and gait[step length,step speed and support phase ratio]were compared between the two groups before and after intervention.Results:After intervention,the WOMAC,HADS scores and support phase ratio of the two groups of patients were significantly lower than before intervention,and the active group was lower than the passive group(P<0.05).Lysholm knee joint score,step length and step speed were significantly higher in both groups than before intervention,and the active group was higher than the passive group(P<0.05).Conclusions:Active exercise intervention can effectively relieve pain in patients with KOA and promote knee function and gait recovery.
