预警预控理念指导下策略优化管理模式对口腔癌术后患者的影响

Effect of Strategic Optimization Management Model under the Guidance of Rarly Warning and Pre-control Concept on Postoperative Patientswith Oral Cancer

目的:探讨预警预控理念指导下策略优化管理模式对口腔癌术后患者依从性、口腔清洁度的影响。方法:选取2020年11月1日~2022年10月31日收治的80例行口腔癌根治术的患者,按照随机数字表法分为对照组和观察组各40例。对照组给予常规管理模式,观察组给予预警预控理念指导下策略优化管理模式,所有患者均干预至出院;比较干预前与干预1周后两组口腔功能(采用改良Beck口腔评分法)、吞咽功能(采用洼田饮水试验)、口腔清洁度、治疗依从性、术后恢复情况、并发症发生情况。结果:干预1周后,两组改良Beck口腔评分均低于干预前(P<0.05),且观察组低于对照组(P<0.05);干预1周后,观察组吞咽功能、口腔清洁度优于对照组(P<0.05);观察组治疗依从性高于对照组(P<0.05);观察组首次经口进食时间、口腔黏膜愈合时间、住院时间及并发症总发生率均优于对照组(P<0.05)。结论:预警预控理念指导下策略优化管理模式能有效提升口腔癌患者术后吞咽功能,提高口腔清洁度与治疗依从性,降低患者术后并发症发生率,利于其预后恢复。

Objective:To explore the effect of strategic optimization management model under the guidance of early warning and precontrol concept on postoperative compliance and oral cleanliness of patients with oral cancer.Methods:A total of 80 patients who underwent radical resection of oral cancer from November 1,2020 to October 31,2022 were selected and divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The routine management model was taken in the control group and the strategic optimization management model was implemented under the guidance of the concept of early warning and pre-control in the observation group.All patients were intervened till discharge.The oral function(using modified Beck Oral assessment scale),swallowing function(Kubota drinking test),oral cleanliness,treatment compliance,postoperative recovery and complications were compared between the two groups before intervention and one week after intervention.Results:After one week of intervention,the Modified Beck Oral Score was lower than that before intervention in the two groups(P<0.05),and that in the observation group was lower than the control group(P<0.05).The swallowing function and oral cleanliness of the observation group were better than those of the control group after one week of intervention(P<0.05).The treatment compliance of the observation group was better than that of the control group(P<0.05).The first oral feeding time,oral mucosal healing time,hospitalization time was shorter and total incidence of complications in the observation group was less than that of the control group(P<0.05).Conclusion:Under the guidance of the concept of early warning and precontrol,the strategic optimization management model can effectively improve the postoperative swallowing function,oral cleanliness and treatment compliance,and can reduce the incidence of postoperative complications of the patients.
