小儿肺炎支原体免疫球蛋白M抗体与超敏C反应蛋白和血清淀粉样蛋白A检验的诊断价值分析

Analysis of the Diagnostic Value of Immunoglobulin M Antibody,High Sensitivity C-reactive Protein and Serum Amyloid a Test in Children With Mycoplasma Pneumoniae

目的 分析小儿肺炎支原体免疫球蛋白M(immunoglobulin M, IgM)与超敏C反应蛋白(hypersensitive Creactive protein, hs-CRP)和血清淀粉样蛋白A(serum amyloid A, SAA)联合检测的诊断价值。方法 非随机选取2022年1月-2023年6月兖州区人民医院收治的肺炎支原体肺炎患儿及同期健康体检儿童各110例,肺炎支原体肺炎患儿为观察组,健康体检儿童作为参照组。对比两组肺炎支原体IgM抗体、hs-CRP、SAA水平检验结果,绘制ROC曲线,分析支原体IgM抗体+hs-CRP与SAA+hs-CRP及联合检验的诊断效能。结果 观察组小儿肺炎支原体IgM抗体水平、hs-CRP水平以及SAA水平比参照组高,差异有统计学意义(P均<0.05);在ROC曲线图中,三者联合检测的AUC值为0.930,高于IgM抗体+hs-CRP与SAA+hs-CRP检测的0.885、0.835,三者联合检测小儿肺炎支原体的灵敏度及特异度高于IgM抗体+hs-CRP与SAA+hs-CRP检测,差异有统计学意义(P均<0.05)。结论 对小儿肺炎支原体肺炎患儿应用肺炎支原体IgM抗体+hs-CRP与SAA三项指标联合检测具有一定价值,具有较高灵敏度、特异度。

Objective To observe the diagnostic value of the combined detection of pediatric Mycoplasma pneumoniae immunoglobulin M(IgM)with hypersensitive C-reactive protein(hs-CRP)and serum amyloid A(SAA).Methods A total of 110 children with mycoplasma pneumoniae pneumonia admitted to Yanzhou District People′s Hospital from January 2022 to June 2023 and no healthy children in the same period were non-randomly selected.The children with mycoplasma pneumoniae pneumonia were selected as the observation group,and the healthy children with physical examination were selected as the reference group.The test results of mycoplasma pneumoniae IgM antibody,hs-CRP and SAA were compared between the two groups,and ROC curve was drawn to analyze the diagnostic efficacy of mycoplasma IgM antibody+hs-CRP and SAA+hs-CRP as well as the combined test.Results The levels of mycoplasma pneumoniae IgM antibody,hs-CRP and SAA in observation group were higher than those in reference group,and the differences were statistically significant(all P<0.05).In the ROC curve,the AUC value of the combined detection of the three components was 0.930,higher than the 0.885 and 0.835 values of IgM+hs-CRP and SAA+hs-CRP.The sensitivity and specificity of the combined detection of the three components were higher than those of IgM+hs-CRP and SAA+hs-CRP in the detection of mycoplasma pneumoniae in children,the differences were statistically significant(both P<0.05).Conclusion The combined detection of Mycoplasma pneumoniae IgM antibody+hs-CRP and SAA is valuable for pediatric patients with Mycoplasma pneumoniae,with high sensitivity and specificity.
