针刺联合吞咽功能训练对脑卒中患者吞咽困难及炎症因子水平的影响

Effect of Acupuncture Combined with Swallowing Training on Swallowing Difficulty and Inflammatory Factors in Stroke Patients

目的:探讨针刺联合吞咽功能训练对脑卒中后吞咽困难患者恢复进度及脑源性神经营养因子的影响。方法:选取2019年6月—2021年4月景德镇市第二人民医院收治的脑卒中后吞咽困难患者90例,采用随机数字表法分为观察组和对照组,各45例。对照组给予针刺治疗,观察组给予针刺联合吞咽功能训练。比较2组治疗前后血清白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF-α)、脑源性神经营养因子(BDNF)和P物质水平,以及2组吞咽功能相关评估量表评分及临床疗效差异。结果:治疗后观察组总有效率、血清BDNF和P物质水平高于对照组,洼田饮水试验、吞咽困难分级及标准吞咽功能评价量表评分低于对照组,血清IL-6、hs-CRP和TNF-α水平降幅大于对照组,组间差异均有统计学意义(P<0.05)。结论:脑卒中后吞咽困难患者应用针刺联合吞咽功能训练,能有效改善患者吞咽功能,降低炎症反应,提高临床疗效,促进患者康复。

Objective:To explore the effect of acupuncture combined with swallowing function training on the recovery progress of dysphagia patients after stroke and on Brain-derived neurotrophic factor.Methods:Ninety patients with dysphagia after stroke who received treatment at the Second People's Hospital of Jingdezhen City,from June 2019 to April 2021 were selected and randomly divided into observation group and control group with 45 cases each.The control group received acupuncture treatment.The observation group was given acupuncture combined with swallowing function training.Compare the serum levels of interleukin-6(IL-6),hypersensitive C-reactive protein(hs CRP),and tumor necrosis factor(TNF-α)Differences in BDNF and substance P,scores of swallowing function related assessment scales between the two groups,and clinical efficacy.Results:After treatment,the total effective rate,serum BDNF and substance P levels in the observation group were higher than those in the control group.The Wada drinking water test,dysphagia grading,and standard swallowing function evaluation scale scores were lower than those in the control group,and serum IL-6,hs CRP,and TNF-αlevels were also lower in the observation group.The decrease in level was greater than that of the control group(P<0.05).Conclusion:Acupuncture combined with swallowing function training can effectively improve swallowing function,reduce inflammatory reactions,improve clinical efficacy,and promote patient recovery in patients with dysphagia after stroke.
