冰0.9%氯化钠溶液含漱联合集束化口腔干预对鼻咽癌放疗患者口腔黏膜炎及睡眠质量的影响

Effect of ice physiological saline gargle combined with cluster oral intervention on oral mucositis and sleep quality in patients with radiotherapy for nasopharyngeal carcinoma

目的研究冰0.9%氯化钠溶液含漱联合集束化口腔干预对鼻咽癌放疗患者口腔黏膜炎及睡眠质量的影响。方法选取2021年1~6月收治的鼻咽癌放疗患者100例按随机数表法分为对照组(行常规护理)及研究组(在对照组基础上采用冰0.9%氯化钠溶液含漱联合集束化口腔干预),每组50例。比较2组患者口腔黏膜炎发生情况、口腔疼痛情况、睡眠质量、心理状况及自我效能。结果研究组口腔黏膜炎总发生率明显低于对照组,差异有统计学意义(P<0.05);干预后研究组疼痛数字评分(NRS)、匹兹堡睡眠质量指数量表(PSQI)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分明显低于对照组,一般自我效能(GSES)评分明显高于对照组,差异有统计学意义(P<0.05)。结论冰0.9%氯化钠溶液含漱联合集束化口腔干预能够减少鼻咽癌放疗患者口腔黏膜炎的发生,改善患者的睡眠质量、口腔疼痛情况及心理状况,同时提高患者的自我效能。

Objective To study the effect of ice physiological saline gargle combined with cluster oral intervention on oral mucositis and sleep quality in radiotherapy for nasopharyngeal carcinoma(NPC).Methods One hundred NPC patients with radiotherapy in our hospital from January 2021 to June 2021 were randomly assigned into study group(n=50,conventional care)and control group(n=50,conventional care+ice physiological saline gargle+cluster oral intervention).The aim was to compare incidence of oral mucositis,oral pain,sleep quality,mental status,self-efficacy.Results The total incidence of oral mucositis was significantly lower in the study group than in the control group(P<0.05).After intervention,significantly lower the scores of Numerical Rating Scale(NRS),Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),and significantly higher General Self-Efficacy Scale(GSES)scores were found in the study group than those of the control group(all P<0.05).Conclusion For NPC patients underwent radiotherapy,ice physiological saline gargle combined with cluster oral intervention can reduce the occurrence of oral mucositis,improve sleep quality,oral pain and psychological status,and enhance self-efficacy.
