临床药师制度在某社区处方点评中的应用效果

Application Effect of Clinical Pharmacist System in a Community Prescription Review

目的:分析临床药师制度在武汉市武昌区水果湖街社区卫生服务中心处方点评中的应用效果。方法:抽取武汉市武昌区水果湖街社区卫生服务中心信息化平台HIS系统中的处方和纸质处方,选取临床药师制度实施前(2022年1-6月)开具的处方1 800张为对照组,选取临床药师制度实施后(2022年7-12月)开具的1 800张处方为观察组。对照组实施常规流程管理,观察组实施临床药师制度。比较两组处方点评合格率,并分析不规范处方和用药不适宜处方情况。结果:观察组处方点评合格率高于对照组(P<0.05)。两组不规范处方类型主要为处方修改不规范、缺医师签名盖章、医师超权限开方,两组不适宜处方类型主要为未按照抗菌药物临床应用管理规定使用抗菌药物、诊断与用药不符、重复用药、药品超过规定用量。结论:临床药师制度在武汉市武昌区水果湖街社区卫生服务中心处方点评中的应用效果较好,能够提升处方合格率,临床药师应针对不规范处方、不适宜处方类型进行合理用药指导,提高整体医疗质量。

Objective:To analyze the application effect of clinical pharmacist system in prescription review of Wuhan Wuchang District Shuiguohu Street Community Health Service Center.Methods:Prescriptions and paper prescriptions were extracted from HIS system of information platform of Wuhan Wuchang District Shuiguohu Street Community Health Service Center.One thousand and eight hundred prescriptions issued before the implementation of the clinical pharmacist system(January 2022 to June 2022)were selected as the control group,and one thousand and eight hundred prescriptions issued after the implementation of the clinical pharmacist system(July 2022 to December 2022)were selected as the observation group.The control group implemented routine process management,and the observation group implemented clinical pharmacist system.The qualified rate of prescription review was compared between the two groups,and the situation of irregular prescription and inappropriate prescription was analyzed.Results:The qualified rate of prescription comments in observation group was higher than that in control group(P<0.05);the two groups of irregular prescription types are mainly irregular prescription modification,lack of doctor's signature and seal,and doctor's authority to prescribe,the two groups of unsuitable prescription types were mainly not using antibiotics according to the clinical application management regulations of antibiotics,diagnosis and drug inconsistency,repeated drug use,drug exceeding the prescribed dosage.Conclusion:The application of clinical pharmacist system in the prescription review of Wuhan Wuchang District Shuiguohu Street Community Health Service Center has a good effect,which can improve the prescription qualification rate.Clinical pharmacists should provide rational drug use guidance for irregular prescriptions and inappropriate prescription types,so as to improve the overall medical quality.
