盐酸利托君联合硫酸镁治疗早产胎膜早破的临床效果分析

Analysis on Clinical Effect of Ritodrine Hydrochloride Injection Combined with Magnesium Sulfate in Treatment of Preterm Premature Rupture of Membranes

目的:探讨盐酸利托君联合硫酸镁治疗早产胎膜早破的临床效果。方法:选取2020年3月-2022年7月北京燕化医院收治的早产胎膜早破患者62例作为研究对象,根据投掷硬币法分为对照组和观察组,各31例。对照组采用硫酸镁治疗,观察组在对照组基础上联合盐酸利托君治疗。对比两组宫缩抑制时间、孕龄延长时间、羊水早破因子水平、炎性因子水平、新生儿Apgar评分、新生儿出生体质量、不良反应发生率。结果:观察组宫缩抑制时间短于对照组,孕龄延长时间长于对照组(P<0.001)。治疗后,两组基质金属蛋白酶9、白细胞介素-6、肿瘤坏死因子-α水平均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组白细胞介素-10、白细胞介素-4水平均低于治疗前,但观察组高于对照组,两组前列腺素E2、一氧化氮水平均低于治疗前,且观察组低于对照组(P<0.05)。观察组新生儿Apgar评分、出生体质量高于对照组(P<0.001)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:盐酸利托君联合硫酸镁治疗早产胎膜早破的临床效果较好,能够缩短子宫抑制时间,延长孕龄,降低炎性因子和羊水早破因子水平,改善新生儿身体状态,且安全性较高。

Objective:To investigate the clinical effect of ritodrine hydrochloride combined with magnesium sulfate in treatment of preterm premature rupture of membranes.Methods:A total of 62 patients with preterm premature rupture of membranes admitted to Beijing Yanhua Hospital from March 2020 to July 2022 were selected as the study subjects.They were divided into control group and observation group by coin throwing method,with 31 cases in each group.The control group was treated with magnesium sulfate and the observation group was treated with ritodrine hydrochloride on the basis of the control group.The time of uterine contraction inhibition,time of gestational age prolongation,levels of amniotic fluid premature rupture factors,levels of inflamma-tory factors,neonatal Apgar score,neonatal birth weight,and incidence of adverse reactions were compared between the two groups.Results:The uterine contraction inhibition time in the observation group was shorter than that in the control group,and the gestational age prolongation time in the observation group was longer than that in the control group(P<0.001).After treatment,the levels of matrix metalloproteinase 9,interleukin-6 and tumor necrosis factor-αin the two groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of interleukin-10 and interleukin-4 in the two groups were lower than those before treatment,but the levels in the observation group were higher than those in the control group,and the levels of prostaglandin E2 and nitric oxide in the two groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group(P<0.05).The Apgar score and neonatal birth weight in the observation group were higher than those in the control group(P<0.001).There was no sig-nificant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ritodrine hydrochlo-ride combined with magnesium sulfate has a good clinical effect in the treatment of preterm premature rupture of membranes,and can shorten the time of uterine contraction inhibition,prolong gestational age,reduce the levels of inflammatory factors and amni-otic fluid premature rupture factors,improve the physical state of newborns,and have high safety.
