多元文化视域下护理人文关怀评价量表的构建与信效度检验

Construction and validity test of evaluation scale of humanistic care in nursing from multicultural perspective

目的:基于多元文化构建适合我国国情的护理人文关怀评价量表。方法:基于多元文化核心理念,通过理论基础、文献检索及质性访谈构建量表条目池,于2022年3月至5月,邀请江苏省、浙江省、安徽省11所三级甲等医院的18名护理管理及临床护理专家进行两轮德尔菲专家函询,构建住院患者护理人文关怀评价量表,通过预调查对问卷进行调整。采用方便抽样法,于2022年9月至10月选取江苏省和安徽省12所三级甲等医院353例患者进行问卷调查,并进行项目分析和信效度检验。结果:两轮专家积极性分别为90%、100%,专家的权威系数为0.85、0.86,熟悉程度系数为0.91、0.94;第一轮专家函询的肯德尔和谐系数为0.171~0.221,第二轮专家函询各级指标的肯德尔和谐系数为0.244~0.253。最终形成住院患者护理人文关怀评价量表指标包括4个一级指标、24个二级指标。问卷的Cronbach'sα系数为0.977,各维度在0.933~0.965;基于标准化项Cronbach'sα系数0.978;总量表效度指数为0.951,各条目效度指数为0.890~0.918;各维度间相关系数为0.542~0.917;探索性因子分析得到4个公因子,累积方差贡献率为63.46%。结论:本研究构建多元文化视域下的住院患者护理人文关怀评价量表科学合理,实用性与操作性强,为促进患者住院体验和护士人文关怀提供参考依据。

Objective:To construct evaluation scale of humanistic care for inpatient nursing based on multi-culture.Methods:Based on the core concept of multiculturism,the scale item pool was constructed with theoretical basis,literature search and qualitative interview.From March to May 2022,18 nursing management and clinical nursing experts from 11 grade-A tertiary hospitals in Jiangsu,Zhejiang and Anhui Provinces were invited to conduct two rounds of Delphi expert correspondence consultation to construct the evaluation scale of humanistic care for inpatient nursing.The questionnaire was adjusted through pre-survey.By convenience sampling method,353 patients from 12 grade-A tertiary hospitals in Jiangsu and Anhui Provinces were selected for questionnaire survey from September to October 2022,and the project analysis and reliability and validity test were conducted.Results:The enthusiasm of the experts in the two rounds was 100%,the authority coefficient of the experts was 0.85,0.86,and the familiarity coefficient was 0.91,0.94.The Kendall harmony coefficient of the first round of expert correspondence consultation was 0.171-0.221,and the Kendall harmony coefficient of the second round of expert correspondence consultation was 0.244-0.253.Finally,the scale of humanistic care needs for inpatient nursing was formed,including 4 first-level indicators and 24 second-level indicators.The Cronbach'sαcoefficient of the questionnaire was 0.977,with each dimension ranging from 0.933 to 0.965.The Cronbach'sαcoefficient was 0.978 based on the standardized item.The validity index of total table was 0.951,and the validity index of each item was 0.890-0.918.The correlation coefficients of each dimension ranged from 0.542 to 0.917.Four common factors were obtained by exploratory factor analysis,and the cumulative variance contribution rate was 63.46%.Conclusion:The humanistic care evaluation scale of inpatient nursing constructed from the perspective of multiculturalism is scientific and reasonable,practical and operable,which provides a reference basis for promoting inpatients' experience and nurses' humanistic care.
