骨髓增生性肿瘤病人JAK-2和CALR基因表达及其与预后关系

摘要 目的探讨骨髓增生性肿瘤(MPN)病人JAK-2和CALR基因表达及其与预后的关系。方法选取2018年7月—2022年7月石家庄市人民医院收治的38例MPN病人纳入研究组,另选取本院无血液系统疾病志愿者40例纳入对照组,采用实时荧光定量PCR技术检测两组骨髓液JAK-2与CALR基因表达。根据预后评价标准将研究组再分为低危组、中危组及高危组3个亚组,比较各亚组病人JAK-2与CALR基因表达,并采用Spearman相关性分析两基因表达与预后的关系。结果相较于对照组,MPN各亚组JAK-2与CALR基因表达显著升高(F=278.674、615.206,P<0.05),其中以原发性血小板增多症的两基因表达最高。随着预后评定级别由低危组到高危组转变,MPN病人两基因表达逐渐升高,且Spearman相关性分析显示,两基因表达均与病人预后呈显著负相关(r=-0.491、-0.503,P<0.01)。结论MPN病人JAK-2与CALR基因表达异常升高,且其表达水平与预后呈负相关,临床可据此评估MPN病人的疗效及预后。 Objective To investigate the expression of the JAK-2 and CALR genes in patients with myeloproliferative neoplasms(MPN)and their association with prognosis.Methods A total of 38 patients with MPN who were admitted to Shijia-zhuang People’s Hospital from July 2018 to July 2022 were enrolled as study group,and 40 volunteers without hematological diseases in our hospital were enrolled as control group.Quantitative real-time PCR was used to measure the expression levels of the JAK-2 and CALR genes in bone marrow fluid.According to the criteria for prognostic evaluation,the patients in the study group were further divided into low-risk subgroup,intermediate-risk subgroup,and high-risk subgroup.The expression levels of the JAK-2 and CALR genes were compared between the three subgroups,and a Spearman correlation analysis was used to investigate the correlation of the expression levels of the JAK-2 and CALR genes with prognosis.Results Compared with the control group,each subgroup of the MPN group had significant increases in the expression levels of the JAK-2 and CALR genes(F=278.674,615.206;P<0.05),with the highest expression levels of the two genes in patients with essential thrombocythemia.For the patients with MPN,there were gradual increases in the expression levels of the JAK-2 and CALR genes from the low-risk group to the high-risk group,and the Spearman correlation analysis showed that the expression levels of the JAK-2 and CALR genes were significantly negatively correlated with the prognosis of patients(r=-0.491,-0.503;P<0.01).Conclusion There are abnormal increases in the expression of the JAK-2 and CALR genes in patients with MPN,and the expression levels of the two genes are negatively correlated with prognosis.Therefore,they can be used to assess the treatment outcome and prognosis of MPN patients in clinical practice.
