老年牙周炎病人睡眠质量特点及其影响因素分析

Characteristics of sleep quality and related influencing factors in elderly patients with periodontitis

目的探讨老年牙周炎病人睡眠质量状况及其影响因素。方法选取59例老年牙周炎(≥65岁)病人作为研究对象,按照匹兹堡睡眠质量指数量表(PSQI)评估睡眠质量。采用单因素分析方法筛选影响老年牙周炎病人睡眠质量的潜在因素,并采用多因素Logistics回归模型分析其独立危险因素。结果59例老年牙周炎病人平均PSQI得分为(7.07±3.55)分;睡眠良好20例(33.9%),睡眠较差39例(66.1%)。单因素分析显示,睡眠良好组和睡眠较差组牙周基础治疗次数、卡氏评分、焦虑抑郁量表评分、多维疲劳量表评分、体育锻炼情况及牙周炎分期比较差异有统计学意义(t=2.35~2.63,χ^(2)=6.17、8.19,P<0.05)。多因素Logistics回归分析显示,体育锻炼情况(β=1.461,OR=4.31,P<0.05)和牙周炎分期(β=1.194,OR=3.30,P<0.05)是影响老年牙周炎病人睡眠质量的独立风险因素。结论老年牙周炎病人普遍存在睡眠质量下降,其独立影响因素包括牙周炎严重程度和是否参加体育锻炼。

Objective To investigate the sleep quality and its influencing factors in elderly patients with periodontitis.Methods A total of 59 elderly patients with periodontitis,aged≥65 years,were enrolled as subjects,and Pittsburgh Sleep Quality Index(PSQI)was used to assess their sleep quality.A univariate analysis was used to obtain the potential influencing factors for sleep quality in elderly patients with periodontitis,and the multivariate Logistic regression model was used to identify independent risk factors for sleep quality.Results Among the 59 elderly patients with periodontitis,there were 20 patients(33.9%)with good sleep quality and 39(66.1%)with poor sleep quality,with a mean PSQI score of 7.07±3.55.The univariate analysis showed that there were significant differences between the good sleep quality group and the poor sleep quality group in the number of basic periodontal treatment sessions,Karnofsky Performance Scale(KPS)score,Hospital Anxiety and Depression Scale(HADS)score,Multi-dimensional Fatigue Catalogue-20(MFI-20)score,physical exercise,and the stage of periodontitis(t=2.35-2.63;χ^(2)=6.17,8.19;P<0.05).The multivariate Logistic regression model showed that the physical exercise(β=1.461,OR=4.31,P<0.05)and the stage of periodontitis(β=1.194,OR=3.30,P<0.05)were independent risk factors for sleep quality in elderly patients with periodontitis.Conclusion There is generally a decline in sleep quality in elderly patients with periodontitis,and the severity of periodontitis and whether to participate in physical exercise are independent influencing factors.
