基层卫生人员关于肌骨关节训练伤的超声快速诊断技术的培训探讨

摘要 目的探讨基层卫生人员关于肌骨关节训练伤的超声快速诊断技术教学培训的可行性和效果。方法培训人员均来自基层医疗卫生单位,共计55名,专业包括康复医学、影像医学、急救医学和骨科,其中超声专业学员20名(占36.36%),非超声医学专业学员35名(占63.64%)。培训内容包括常见肌骨关节损伤(积液、急性肌腱和韧带撕裂)超声诊断技术的理论授课和实际操作手把手教学[1]。理论课程由超声医学专家授课,技能培训由上述专家和超声专业医师实施一对一教学。培训时长20学时。所有学员在培训前填写线上调查问卷,并在培训结束后参加理论考核(百分制)、实际操作考核(等级制)及满意度调查。结果35名非超声专业学员和20名超声专业学员的理论平均成绩分别为(93.11±1.02)分和(93.15±1.30)分,二者相比差异无统计学意义(t=0.126,P=0.800)。表明通过短期培训后,非超声专业的学员与超声专业学员同样可以准确掌握肌骨关节超声快速诊断的理论知识。在实际操作方面,35名非超声专业学员中,优秀24名(占68.57%),良好11名(占31.43%);20名超声专业学员中,优秀15名(占75.00%),良好5名(占25.00%)。两组学员均完成了操作考核,在优秀率上差异无统计学意义(P>0.05)。结论关于对基层卫生人员进行常见肌骨关节训练伤的超声快速诊断技术教学培训具有可行性,能够达到预期效果。 Objective The aim was to explore the feasibility and effectiveness of teaching and training of primary health care personnel on ultrasound rapid diagnostic techniques for musculoskeletal joint training injuries.Methods The participants were 55 doctors and health workers from primary healthcare units,specialising in rehabilitation medicine,imaging medicine,emergency medicine and orthopaedics,including 20 trainees(36.36%)specialising in ultrasound and 35 trainees(63.64%)not specialising in ultrasound medicine.The training included theoretical lectures and practical teaching of ultrasound diagnostic skills for common musculoskeletal and joint injuries(effusion,acute tendon and ligament tears).The theoretical course was taught by ultrasound specialists of associate degree or above,and the skills training was conducted by the above specialists and sonographers skilled in rapid diagnosis of musculoskeletal joint ultrasound on a one-on-one basis.The duration of the training was 20 hours.All trainees fill out an online questionnaire before the training and take a written theoretical knowledge test(percentage system),a practical skills test(grade system)and a satisfaction survey at the end of the training.Finally,we statistically analysed the collected data.Results All 55 trainees participated in the whole training.35 non-ultrasound specialised trainees and 20 ultrasound specialised trainees scored(93.11±1.02)and(93.15±1.30)mean theoretical scores,respectively,with no statistically significant difference(P>0.05).The above results showed that there was no statistical difference between the non-ultrasound specialised trainees and the ultrasound specialised trainees in the mastery of the theoretical knowledge of rapid diagnosis of musculoskeletal joint ultrasound(the content of the discussion).In terms of practical skills,of the 35 non-ultrasound students,24(68.57%)were excellent and 11(31.43%)were good;of the 20 students with basic imaging skills,15(75.00%)were excellent and 5(25.00%)were good.Both groups of trainees completed the operational assessment,and there was no statistically significant difference in the excellence rate(P>0.05).The pretraining and post-training satisfaction questionnaires showed that the training met the training expectations of the trainees,with an overall satisfaction rate of 98.2%.Conclusion The teaching and training of grassroots health personnel on ultrasound rapid diagnostic techniques for common musculoskeletal joint training injuries is feasible and achieves the expected results.
