摘要
目的宫颈癌患者人乳头瘤病毒(HPV)感染情况对癌组织Run相关转录因子3(Runx3)、NIN1/RPN12结合蛋白1同源物(NOB1)、神经激肽-1受体(NK-1R)表达水平的影响及其检测价值分析。方法选取本院在2019年2月~2024年2月进行住院治疗的109例宫颈癌HPV感染患者作为研究组,依据HPV感染检查结果,将其分为高危型组(n=62)、低危型组(n=47)。同期选取在本院就诊的100例宫颈癌无HPV感染患者作为对照组。采用qRT-PCR法分别检测癌组织中Runx3、NOB1、NK-1R表达水平;多因素Logistic回归分析影响宫颈癌患者HPV感染情况的相关因素;运用ROC曲线分析癌组织Runx3、NOB1、NK-1R对宫颈癌患者高危型HPV感染的诊断价值。结果与对照组比较,研究组NOB1、NK-1R表达水平显著升高,Runx3表达水平显著下降(P<0.05);高危型组发生淋巴结转移占比、低分化程度占比、NOB1、NK-1R表达水平显著高于低危型组,而Runx3表达水平低于低危型组(P<0.05);淋巴结转移、分化程度、Runx3、NOB1、NK-1R为宫颈癌患者高危型HPV感染的影响因素(P<0.05);Runx3、NOB1、NK-1R联合诊断宫颈癌患者高危型HPV感染的曲线下面积(AUC)为0.946,优于各自单独诊断(Z_(三者联合-Runx3)=2.113、Z_(三者联合-NOB1)=2.634、Z三者联合-NK-1R=2.538,P=0.035、0.008、0.011),其敏感度及特异度分别为95.16%、89.36%。结论宫颈癌HPV感染患者及其高危型患者NOB1、NK-1R表达水平显著升高,Runx3表达水平显著降低,三者联合对宫颈癌患者高危型HPV感染有更高的诊断价值。
Objective To analyze the impacts of human papillomavirus(HPV)infection on the expression levels of Run related transcription factor 3(Runx3),NIN1/RPN12 binding protein 1 homolog(NOB1),and neurokinin-1 receptor(NK-1R)in cervical cancer patients and their detection value.MethodsA total of 109 cervical cancer patients with HPV infection who were hospitalized in our hospital from July 2024 to February 2019 were selected as the study group.Based on the results of HPV infection tests,they were grouped into high-risk group(n=62)and low-risk group(n=47).Meanwhile,loo cervical cancer patients without HPV infection who visited our hospital were selected as the control group.The qRT-PCR method was applied to detect the expression levels of Runx3,NOB1,and NK-1R in cancer tissues.Multivariate logistic regression was applied to analyze the relevant factors affecting HPV infection in cervical cancer patients.ROC curve was applied to analyze the diagnostic value of Runx3,NOB1,and NK-1R for high-risk HPV infection in cervical cancer patients.Results Compared with the control group,the expression levels of NOBl and NK-1R in the study group were obviously higher,while the expression level of Runx3 was obviously lower(P<0.05).The proportion of lymph node metastasis,the proportion of low differentiation,and the expression levels of NOB1 and NK-1R in the high-risk group were obviously higher than those in the low-risk group,while the expression level of Runx3 was lower than that in the low-risk group(P<0.05).Lymph node metastasis,degree of differentiation,Runx3,NOB1,and NK-1R were the influencing factors of high-risk HPV infection in cervical cancer patients(P<0.05).The area under the curve(AUC)of Runx3,NOB1,and NK-1R combined diagnosis for high-risk HPV infection in cervical cancer patients was 0.946,which was better than their individual diagnosis(Z_(triple combination-Runx3)=2.113,Z_(triple combination-NOB1)=2.634,Z_(triple combination-NK-1R)=2.538,P=0.035,0.008,0.011),with sensitivity and specificity of 95.16%and 89.36%,respectively.Conclusion The expression levels of NOBl and NK-1R are obviously elevated in HPV infected cervical cancer patients and high-risk patients,and the expression level of Runx3 is obviously reduced.The combination of the three has higher diagnostic value for high-risk HPV infection in cervical cancer patients.
