FOCUS-PDCA程序管理在降低高原地区围产期并发症的应用

Application of FOCUS-PDCA Program Management in Reducing Perinatal Complications in Plateau Area

目的:分析通过FOCUS-PDCA程序管理对于提高产科质量,降低围产期母儿并发症,保障母婴安全的效果。方法:选取青海省刚察县人民医院2021年1月—2022年2月住院分娩的222例孕产妇为对照组,2022年3—12月住院分娩的248例孕产妇为干预组。对照组采用常规护理方式,干预组在常规护理措施的基础上增加FOCUS-PDCA程序管理。比较两组孕产妇围产期并发症及妊娠结局、新生儿并发症。结果:两组孕妇妊娠期高血压和院外分娩发生率比较,差异无统计学意义(P>0.05),但两组在早产、胎膜早破、贫血、产后出血方面,差异有统计学意义(P<0.05)。两组孕妇分娩方式、死胎、新生儿出生体质量比较,差异无统计学意义(P>0.05),但两组难产、新生儿Apgar评分比较,差异有统计学意义(P<0.05)。两组巨大儿、出生缺陷发生率比较,差异无统计学意义(P>0.05),低体质量儿、新生儿窒息发生率相比差异具有统计学意义(P<0.05)。结论:FOCUS-PDCA程序管理模式可以显著降低围产期母儿并发症发生率,改善妊娠结局,产科安全质量得到提高。

Objective:To analyze the effects of FOCUS-PDCA program management on improving obstetric quality,reducing perinatal complications and ensuring maternal and infant safety.Methods:A total of two hundred and twenty-two pregnant women who delivered in hospital from January 2021 to February 2022 in Qinghai Gangcha County People's Hospital were selected as the control group,and two hundred and forty-eight pregnant women who delivered in hospital from March 2022 to December 2022 were selected as the intervention group.The control group received routine nursing,and the intervention group received FOCUS PDCA program management based on routine nursing measures.Perinatal complications,pregnancy outcomes and neonatal complications were compared between the two groups.Results:There were no statistically significant differences in the incidence of hypertension and out-of-hospital delivery between the two groups(P>0.05),but there were statistically significant differences in preterm delivery,premature rupture of membranes,anemia and postpartum hemorrhage between the two groups(P<0.05);there were no significant differences in delivery mode,stillbirth and neonatal birth weight between the two groups(P>0.05),but there were significant differences in dystosis and neonatal Apgar score between the two groups(P<0.05);there were no significant differences in the incidence of macrosomia and birth defects between the two groups(P>0.05),while there were significant differences in the incidence of low body weight infants and neonatal asphyxia between the two groups(P<0.05).Conclusion:FOCUS-PDCA program management model can significantly reduce the incidence of perinatal maternal and infant complications,improve pregnancy outcomes,and improve the quality of obstetric safety.
