摘要
目的:探讨运动训练联合针灸在重症监护病房(ICU)机械通气慢性阻塞性肺疾病(COPD)患者中的应用效果。方法:选取2021年6月—2023年6月在广西壮族自治区人民医院ICU治疗的40例有创机械通气COPD患者,采用随机数字表法随机分为观察组和对照组两组,每组20例。观察组采用运动训练联合针灸治疗;对照组仅采用运动训练治疗。比较两组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧合指数及机械通气时间、撤机成功率。结果:治疗前,两组PaO_(2)、PaCO_(2)和氧合指数比较,差异无统计学意义(P>0.05);治疗1个疗程后,观察组PaO_(2)和氧合指数高于对照组,PaCO_(2)低于对照组,差异有统计学意义(P<0.05)。观察组机械通气时间短于对照组,差异有统计学意义(P<0.05)。两组撤机成功率比较,差异无统计学意义(P>0.05)。结论:运动训练联合针灸应用于ICU机械通气COPD患者,能改善呼吸功能,缩短机械通气时间。
Objective To investigate the application effect of exercise training combined with acupuncture in mechanical ventilation of patients with chronic obstructive pulmonary disease(COPD)in intensive care unit(ICU).Methods A total of 40 COPD patients with invasive mechanical ventilation treated in the ICU of the People's Hospital of Guangxi Zhuang Autonomous Region from June 2021 to June 2023 were selected and randomly divided into observation group and control group by random number table method,with 20 cases in each group.The observation group was treated with exercise training combined with acupuncture.The control group was treated with exercise training.Arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygenation index,mechanical ventilation time and success rate of removal were compared between the two groups.Results Before treatment,there was no significant difference in PaO_(2),PaCO_(2)and oxygenation index between the two groups(P>0.05).After 1 course of treatment,PaO_(2)and oxygenation index of observation group were higher than control group,PaCO_(2)was lower than control group,the difference was statistically significant(P<0.05).The mechanical ventilation time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the success rate of removal between the two groups(P>0.05).Conclusions Exercise training combined with acupuncture can improve the respiratory function and shorten the mechanical ventilation time in COPD patients with mechanical ventilation in ICU.
