摘要
目的 探讨叉头转录蛋白O亚族3(FOXO3)、硫氧还蛋白相互作用蛋白(TXNIP)在慢性心力衰竭患者血清中的表达及联合动态心电图检查对慢性心力衰竭的诊断价值。方法 前瞻性选取2022年6月至2023年1月期间于湖北省鄂州市中心医院收治的68例慢性心力衰竭患者作为心力衰竭组,另选取68例同期门诊健康体检者为对照组。比较两组血清FOXO3,TXNIP蛋白水平;ROC曲线分析血清FOXO3、TXNIP对慢性心力衰竭的诊断效能;四表格法分析动态心电图联合血清FOXO3,TXNIP对慢性心力衰竭的诊断效能。结果 心力衰竭组血清FOXO3蛋白表达水平为(1.28±0.34)ng/ml低于对照组(1.73±0.36)ng/ml(P<0.05),TXNIP蛋白表达水平为(126.42±12.98)pg/ml高于对照组(108.86±12.73)pg/ml,差异有统计学意义(P<0.05)。动态心电图联合血清FOXO3,TXNIP诊断慢性心力衰竭的准确率为94.85%,敏感性为97.06%,特异性为92.65%;三项联合诊断的准确率和敏感性均高于单独诊断(P<0.05)。结论 慢性心力衰竭患者血清FOXO3表达水平降低,TXNIP表达水平升高,动态心电图联合血清FOXO3,TXNIP蛋白水平对慢性心力衰竭具有较高的诊断价值。
Objective To investigate the expressions of serum forkhead box protein O3(FOXO3)and thioredoxin interacting protein(TXNIP)and value of Holter combined with FOXO3 and TXNIP in diagnosis of chronic heart failure(CHF).Methods CHF patients were prospectively chosen into CHF group and healthy controls were chosen into control group(each n=68)from Central Hospital of Ezhou City of Hubei Province from June 2022 to Jan.2023.The levels of FOXO3 and TXNIP were compared between 2 groups.The diagnostic efficacy of FOXO3 and TXNIP in CHF diagnosis was analyzed by using ROC curve.The diagnosis of Holter combined with FOXO3 and TXNIP to CHF was analyzed by using four-grid table method.Results The expression of FOXO3(1.28±0.34)ng/ml was lower in CHF group than that in control group(1.73±0.36)ng/ml(P<0.05),and expression of TXNIP(126.42±12.98)pg/ml was higher in CHF group than that in control group(108.86±12.73)pg/ml(P<0.05).The accuracy rate of Holter combined with FOXO3 and TXNIP was 94.85%,sensitivity was 97.06%and specificity was 92.65%in diagnosing CHF.The accuracy rate and sensitivity of Holter combined with FOXO3 and TXNIP were higher than those of Holter,FOXO3 or TXNIP alone(P<0.05).Conclusion The expression of serum FOXO3 decreases and expression of serum TXNIP increases in CHF patients.Holter combined with FOXO3 and TXNIP has a higher diagnostic value to CHF.
