miR-27a、miR-101联合c-erb-B2检测在胃癌术后复发中的预测价值

Predictive value of miR⁃27a and miR⁃101 combined with c⁃erb⁃B2 in postoperative recurrence of gastric cancer

摘要 目的探究miR-27a、miR-101联合c-erb-B2检测在胃癌术后复发中的预测价值。方法选取2021年12月至2022年12月芜湖市第二人民医院收治的152例原发性胃癌患者为研究对象,均进行手术治疗。选取152例胃癌患者的胃癌组织和距肿瘤边缘4 cm的癌旁组织,比较胃癌组织和癌旁组织miR-27a、miR-101和c-erb-B2表达;术后随访1年,根据术后复发情况分为复发组(n=27)和未复发组(n=125),比较两组不同临床特征和miR-27a、miR-101、c-erb-B2表达;分析胃癌术后复发的危险因素;采用ROC曲线评估miR-27a、miR-101、c-erb-B2表达单一及联合对胃癌患者术后复发的预测价值。结果胃癌组织miR-27a表达水平高于癌旁组织,miR-101表达水平低于癌旁组织,c-erb-B2阳性表达率高于癌旁组织,差异有统计学意义(P<0.05);复发组和未复发组性别、年龄、肿瘤原发部位、分化程度比较,差异无统计学意义(P>0.05);两组肿瘤直径、TNM分期、浆膜浸润、淋巴结转移、c-erb-B2、miR-101和miR-27a表达比较,差异有统计学意义(P<0.05);二元Logistic回归分析结果显示,肿瘤直径≥5 cm、TNM分期为Ⅲ期、有淋巴结转移、有浆膜浸润、c-erb-B2表达阳性、miR-101低水平表达、miR-27a高水平表达均是影响胃癌患者复发的独立危险因素(P<0.05);ROC曲线显示,miR-27a、miR-101、c-erb-B2联合检测预测胃癌患者复发的AUC为0.906,高于三者单独检测的0.729、0.803、0.834。结论miR-27a水平升高、miR-101水平下降以及c-erb-B2表达阳性均是胃癌术后复发的危险因素,三者联合检测在预测胃癌术后复发中具有良好的应用价值,可为临床诊治提供参考依据。 Objective To investigate the predictive value of miR-27a and miR-101 combined with c-erb-B2 detection in postoperative recurrence of gastric cancer.Methods 152 primary gastric cancer patients who were admitted to The Second People's Hospital of Wuhu from December 2021 to December 2022 were se-lected for the study.All of them underwent surgical treatment.Gastric cancer tissues and paracancerous tis-sues≥4 cm from the tumor margins of 152 gastric cancer patients were selected to compare the expression of miR-27a,miR-101,and c-erb-B2 in gastric cancer tissues and paracancerous tissues.Followed up for 1 year af-ter surgery,the patients divided into the recurrence group(n=27)and the non-recurrence group(n=125)based on their postoperative recurrence status.The two groups were compared in terms of their different clinical characteristics and miR-27a,miR-101,and c-erb-B2 expressions.Different risk factors for postoperative recur-rence of gastric cancer patients were analyzed using logistic regression.The ROC curve was used to assess the risk factors for postoperative recurrence of gastric cancer patients.Logistic regression was used to analyze the risk factors for postoperative recurrence of gastric cancer.While ROC curves were used to assess the predic-tive value of miR-27a,miR-101,c-erb-B2 expression for postoperative recurrence in gastric cancer patients.Results Gastric cancer tissues had higher miR-27a expression levels than paracancerous tissues,lower miR-101 expression levels than paracancerous tissues,and more c-erb-B2 positive expression rate was higher than para-cancerous tissues(P<0.05).There was no statistically significant difference between the recurrence group and the non-recurrence group in terms of gender,age,primary site of tumor,and degree of differentiation(P>0.05).The differences between the two groups in terms of tumor diameter,TNM stage,plasma membrane in-filtration,lymph node metastasis,and expression of c-erb-B2,miR-101 and miR-27a were statistically signifi-cant(P<0.05).Logistic regression analysis showed that tumor diameter≥5 cm,TNM stageⅢ,presence of lymph node metastasis,presence of plasma membrane infiltration,positive expression of c-erb-B2,low-level expression of miR-101,and high level of miR-27a expression were all independent risk factors affecting the re-currence of gastric cancer patients(P<0.05).The ROC curve showed that the AUC of miR-27a,miR-101,and c-erb-B2 combined assay for predicting recurrence in gastric cancer patients was 0.906,which was higher than that of the three individual assays of 0.729,0.803,and 0.834.Conclusion Elevated miR-27a level,de-creased miR-101 level and positive c-erb-B2 expression are risk factors for gastric cancer recurrence after sur-gery.The combined detection of the three has good application value in predicting the recurrence of gastric can-cer after surgery.This provides a better reference for clinical diagnosis and treatment.
