呼吸训练对脑卒中患者上肢功能及平衡能力的影响

摘要 目的观察呼吸训练对脑卒中患者上肢功能及平衡能力的影响。方法选取脑卒中患者64例, 按照随机数字表法将其分为对照组和试验组, 每组32例。2组患者均给予常规康复治疗(包括物理因子治疗、肢体运动训练、作业治疗), 试验组在此基础上增加呼吸训练。2组患者均每日治疗1次, 每周5 d, 连续4周。治疗前和治疗4周后(治疗后), 采用Fugl-Meyer运动功能量表上肢部分(FMA-UE)、上肢运动研究量表(ARAT)评定2组患者的上肢功能, 采用Berg平衡量表(BBS)、躯干控制能力测试(TCT)评定2组患者的平衡能力, 采用改良Barthel指数(MBI)评定2组患者的日常生活活动(ADL)能力。结果治疗后, 2组患者的FMA-UE、ARAT、BBS、TCT、MBI评分均较组内治疗前改善(P<0.05), 且试验组治疗后FMA-UE[(44.56±8.69)分]、ARAT[(42.50±11.68)分]、BBS[(39.34±7.74)分]、TCT[(70.68±19.40)分]、MBI[(80.00±15.45)分]评分优于对照组(P<0.05)。结论呼吸训练能有效改善脑卒中患者的上肢功能及平衡能力。 Objective:To explore any effect of respiratory training on the upper limb functioning and balance of stroke survivors.Methods:Sixty-four stroke survivors were randomly divided into an experimental group and a control group,each of 32.Both groups were given routine rehabilitation treatment(including physical agent therapy,physical training and occupational therapy),but the experimental group also received respiratory training once a day,5 days a week for 4 weeks.Before and after the treatment,upper limb function was evaluated using Fugl-Meyer Upper Extremity Scale(FMA-UE)and the Action Research arm test(ARAT).The subjects′balance was quantified using the Berg Balance Scale(BBS)and the trunk control test(TCT).Ability in the activities of daily living(ADL)was assessed using the modified Barthel index(MBI).Results:After 4 weeks of treatment,the average FMA-UE,ARAT,BBS,TCT and MBI scores of both groups had increased significantly,but the experimental group′s averages were then all significantly better than those of the control group.Conclusions:Respiratory training can promote the recovery of upper limb function and balance after a stroke.
