Professor Zhan Qiang's Experience in The Treatment of Enuresis in Children with Deficiency Cold Type Based on The Principle of"Three Parts and Three Layers"

小儿遗尿多由阳气虚弱,下元虚寒,膀胱不得温煦,尿道失司所致。治疗原则是激发自身阳气,膀胱固涩如常,阴阳平秘。治疗需按照“三部三层”即天、人、地部以及皮、肉、骨三层。天部指根据自然界的阳气变化规律,选择最佳的治疗时间和季节;人部以推拿治疗为主,需根据病情轻重,选择合适皮、肉、骨三层的手法和刺激强度;地部指遗尿着重调理肾心二脏,可借汤药达到固本温元,宁心安神之效。本文为中医治疗小儿遗尿提供了一种临证思路。

Pediatric enuresis is often caused by deficiency in Yang energy,coldness in the lower body,and inability of the bladder to be warmed,leadingto dysfunction in urine control.The principle of treatment is to stimulate the body's own Yang energy,ensure the bladder's proper function,andbalance Yin and Yang.Treatment should follow the"Three Parts,Three Levels"principle,namely Heaven,Human,and Earth parts,as well as theskin,flesh,and bone levels.The Heaven part involves choosing the optimal treatment time and season based on natural Yang energy changes;the Humanpart primarily uses massage therapy,selecting the appropriate techniques and stimulation intensity for the skin,flesh,and bone levels based onthe severity of the condition;the Earth part focuses on regulating the kidney and heart,using herbal medicines to strengthen the foundation,warm thebody,and calm the mind.This article provides a clinical approach for the traditional Chinese medicine treatment of pediatric enuresis.
