摘要
目的:比较静态导航技术引导根管治疗与传统显微根管治疗在钙化根管疏通的精准度。方法:选取56颗离体单根前牙和前磨牙作为研究对象。模拟钙化根管形态后随机分为两组,并按照牙弓形态排列在树脂基托模型中。实验组(n=28)应用静态导航技术引导根管治疗,对照组(n=28)应用传统显微根管治疗。治疗前后分别进行锥束计算机体层技术(cone beam computer tomography,CBCT)扫描以及数字化光学表面扫描,比较两组根管治疗时长、牙体硬组织损失量以及根管偏差值。结果:对照组有4颗离体牙经根管治疗后发生了侧穿或者根尖偏移。实验组与对照组的平均治疗时长分别为(6.92±1.65)min和(14.67±3.72)min,差异有统计学意义(P<0.05)。实验组与对照组的平均牙体硬组织损失量为(10.58±1.50)mm3和(12.95±2.88)mm3,差异有统计学意义(P<0.05)。根管偏差分析,实验组与对照组的根向差(0.40±0.10 mm vs 0.48±0.15 mm)和轴向角度(2.04±0.48°vs 4.59±1.89°)比较,差异均有统计学意义(P<0.05);实验组(0.29±0.06 mm)与对照组的冠向差(0.33±0.08 mm)比较,差异则无统计学意义(P>0.05)。结论:静态导航技术引导根管治疗的根向差和轴向角度比传统显微根管治疗小,治疗时间短,牙体硬组织损失量少。静态导航技术引导根管治疗在单根非弯曲钙化根管的疏通方面优于传统的显微根管治疗。
Objective:To compare the accuracy of static navigation technology-guided root canal therapy and traditional microscopic root canal therapy in clearing single-rooted non-curved calcified root canals.Methods:This study selected a total of 56 single-rooted anterior teeth and premolars extracted due to orthodontic or periodontal diseases at the oral outpatient clinic as the research subjects.After simulating the morphology of calcified root canals,the teeth were randomly divided into two groups and arranged in resin base models according to the shape of the dental arch.In the experimental group(n=28),utilizing static navigation technology for root canal therapy,while the control group(n=28),utilizing traditional microscopic root canal therapy.CBCT scans and digital optical surface scans were performed before and after treatment,and the duration of root canal treatments,the amount of hard tissue loss,and the deviation values of the root canals between the two groups were compared.Results:In this experiment,four teeth in the control group experienced lateral perforation or apical deviation.The average treatment duration for the experimental group and the control group was(6.92±1.65)min and(14.67±3.72)min respectively,the difference was statistically significant(P<0.05).The average amount of hard tissue loss for the experimental group and the control group was(10.58±1.50)mm~3 and(12.95±2.88)mm~3,respectively,the difference was statistically significant(P<0.05).The differences in apical deviation(0.40±0.10 mm vs 0.48±0.15 mm)and axial angle(2.04±0.48°vs 4.59±1.89°)between the experimental and control groups were statistically significant(P<0.05),respectively,while the difference in coronal deviation(0.29±0.06 mm vs 0.33±0.08 mm)was not statistically significant(P>0.05).Conclusion:The differences in apical deviation and axial angle of root canal therapy guided by static navigation technique are smaller than those of traditional microscopic root canal therapy,the treatment time is shorter,and the loss of tooth hard tissue is less.Root canal therapy guided by static navigation technique is superior to traditional microscopic root canal therapy in the dredging of single non-curved calcified root canals.
