加速康复外科理念在膝关节镜下半月板修复围手术期护理中的应用研究

Study on the Application of Accelerated Rehabilitation Surgical Concepts in Perioperative Nursing Care of Knee Arthroscopic Meniscus Repair

目的:研究加速康复外科(ERAS)护理在膝关节镜下半月板修复围手术期护理中的应用效果。方法:对于2020年1—8月于滨州医学院附属医院骨关节外科一病区门诊关节镜手术室进行膝关节镜下半月板修复手术的100例患者分组研究,随机分为ERAS护理组和传统护理组。比较两组患者围手术期视觉模拟评分法(VAS)疼痛评分、膝关节活动度恢复时间、首次下床活动时间、首次踝泵功能锻炼时间、住院时间、住院费用的差异。结果:术前ERAS护理组VAS评分与传统护理组比较,差异无统计学意义(P>0.05);术后两组VAS评分均降低,但ERAS护理组低于传统护理组,差异有统计学意义(P<0.05)。ERAS护理组的术后膝关节活动度大于传统护理组(P<0.05)。ERAS护理组的术后首次下床活动时间及首次踝泵功能锻炼时间早于传统护理组(P<0.05)。ERAS护理组的平均住院日及住院费用低于传统护理组(P<0.05)。ERAS护理组患者满意度优于传统护理组(P<0.05)。结论:ERAS护理在膝关节镜下半月板修复手术期护理的应用效果理想。

Objective:To study the application effect of accelerated rehabilitation surgical(ERAS)care in perioperative care of knee arthroscopic meniscus repair.Methods:One hundred patients who underwent knee arthroscopic meniscus repair surgery in the outpatient arthroscopy operating room of the first ward of the Department of Bone and Joint Surgery,Affiliated Hospital of Binzhou Medical College from January to August 2020 were grouped and studied,and randomly divided into the ERAS nursing group and the traditional nursing group.The differences in perioperative visual analog scale(VAS)pain scores,knee mobility recovery time,first time out of bed,first time of ankle pump functional exercise,hospitalization time,and hospitalization costs were compared between the two groups.Results:Preoperative VAS scores in the ERAS nursing group were not statistically significant compared with those in the traditional nursing group(P>0.05);postoperative VAS scores were reduced in both groups,but the ERAS nursing group was lower than that in the traditional nursing group,and the difference was statistically significant(P<0.05);postoperative knee mobility in the ERAS nursing group was greater than that in the traditional nursing group(P<0.05);the time of the first postoperative out-of-bed activity and the time of the first ankle pump functional exercise were earlier in the ERAS nursing group than in the traditional nursing group(P<0.05);the average hospitalization day and hospitalization cost in the ERAS nursing group were lower than those in the traditional nursing group(P<0.05);and the patient satisfaction in the ERAS nursing group was better than that in the traditional nursing group(P<0.05).Conclusion:The application of ERAS nursing in the surgical period of knee arthroscopic meniscus repair is ideal.
