基于知信行理论的透析中卧位运动康复护理在慢性肾脏病合并衰弱患者维持性血液透析中的应用效果分析

Analysis on Application Effect of Supine Exercise Rehabilitation Care During Dialysis Based on Knowledge Attitude-Practice Theory in Maintenance Hemodialysis of Patients with Chronic Kidney Disease and Frailty

目的:分析基于知信行理论(KAP)的透析中卧位运动康复护理在慢性肾脏病(CKD)合并衰弱患者维持性血液透析(MHD)中的应用效果。方法:选取2022年10月—2023年6月于青岛大学附属医院行MHD治疗的CKD合并衰弱患者90例作为研究对象,使用随机数字表法分为干预组和对照组,各45例。对照组实施常规血液透析护理,干预组在对照组基础上实施基于KAP的透析中卧位运动康复护理。比较两组护理效果。结果:护理后,干预组衰弱评分低于护理前及同期对照组(P<0.001)。护理后,两组挫折评分比较,差异无统计学意义(P>0.05);干预组躯体症状、疲劳、抑郁情绪、人际关系评分及总分高于护理前及同期对照组(P<0.001)。结论:基于KAP的透析中卧位运动康复护理在CKD合并衰弱患者MHD中的应用效果显著,可改善患者衰弱状态,提高生活质量。

Objective:To analyze the application effect of supine exercise rehabilitation care during dialysis based on knowledge-attitude-practice(KAP)theory in maintenance hemodialysis(MHD)of patients with chronic kidney disease(CKD)and frailty.Methods:A total of 90 patients with CKD and frailty who underwent MHD treatment in the Affiliated Hospital of Qingdao University from October 2022 to June 2023 were selected as the study subjects.They were divided into intervention group and control group using random number table method,with 45 patients in each group.The control group received routine hemodialysis care,while the intervention group received the KAP-based supine exercise rehabilitation care on the basis of control group.The care effect was compared between the two groups.Results:After care,the frailty score in the intervention group was lower than that before care and that in the control group during the same period(P<0.001).After care,there was no significant difference in frustration scores between the two groups(P>0.05);The scores of somatic symptoms,fatigue,depression mood,interpersonal relationship and total scores in the intervention group were higher than those before care and those in the control group during the same period(P<0.001).Conclusion:The KAP-based supine exercise rehabilitation care during dialysis has significant application effect in the MHD of patients with CKD and frailty,can help improve frailty status of patients and enhance quality of life.
