摘要
目的探究血清血小板源性生长因子(PDGF)、C1q肿瘤坏死因子相关蛋白9(CTRP9)、沉默信息调节因子1(SIRT1)水平与急性脑梗死患者病情的相关性及临床意义。方法回顾性选取焦作市第二人民医院2020年2月—2021年2月急性脑梗死患者96例(脑梗死组)及健康体检者96名(对照组),依据患者神经功能缺损程度分为轻度患者、中度患者、重度患者,并根据患者3个月后预后情况分为预后不良者、预后良好者。对比入院时两组、不同病情程度、不同预后患者血清PDGF、CTRP9、SIRT1水平,分析各指标与病情的相关性及预后的预测价值。结果脑梗死组血清PDGF、CTRP9、SIRT1水平低于对照组(P<0.05);重度患者血清PDGF、CTRP9、SIRT1水平低于中度、轻度患者,中度患者低于轻度患者(P<0.05);各指标血清水平与急性脑梗死患者病情程度呈负相关(P<0.05);预后良好者血清PDGF、CTRP9、SIRT1水平高于预后不良者(P<0.05);各指标联合检测急性脑梗死预后不良的AUC值为0.856(P<0.05)。结论血清PDGF、CTRP9、SIRT1水平与急性脑梗死患者病情程度密切相关,各指标联合检测对急性脑梗死患者预后预测具有一定参考价值。
Objective To explore the correlation and clinical significance of the serum levels of platelet-derived growth factor(PDGF),C1q tumor necrosis factor related protein 9(CTRP9),silent information regulator 1(SIRT1)with the conditions of patients with acute cerebral infarction.Methods 96 patients with acute cerebral infarction(cerebral infarction group)and 96 healthy people(control group)in Second People's Hospital of Jiaozuo from February 2020 to February 2021 were selected retrospectively,the patients were divided into mild patients,moderate patients and severe patients according to the degree of neurological impairment.According to the prognosis after 3 months,the patients were divided into patients with poor prognosis and patients with good prognosis.The serum levels of PDGF,CTRP9 and SIRT1 were compared between the two groups at admission,in patients with different disease severity and different prognosis,and the correlation between each index and disease condition and the predictive value of prognosis were analyzed.Results Serum levels of PDGF,CTRP9 and SIRT1 in the cerebral infarction group were lower than those in the control group(P<0.05).Serum levels of PDGF,CTRP9 and SIRT1 in severe patients were lower than those in moderate and mild patients,and those in moderate patients were lower than those in mild patients(P<0.05).Serum levels of each index were negatively correlated with the severity of acute cerebral infarction(P<0.05).Serum levels of PDGF,CTRP9 and SIRT1 in patients with good prognosis were higher than those in patients with poor prognosis(P<0.05).AUC value of the combined detection of various indicators for poor prognosis of acute cerebral infarction was 0.856(P<0.05).Conclusion Serum levels of PDGF,CTRP9 and SIRT1 are closely related to the severity of patients with acute cerebral infarction.The combined detection of these indicators has a certain reference value for the prognosis of patients with acute cerebral infarction.
