针刺八髎穴联合康妇消炎栓对中度宫腔粘连分离术后子宫内膜及血流动力学的影响

Effects of Acupuncture at Baliao Points Combined with Kangfu Xiaoyan Suppository on Endometrium and Hemodynamics after Moderate Intrauterine Adhesion Separation Surgery

目的探究针刺八髎穴联合康妇消炎栓对中度宫腔粘连(intrauterine adhesion,IUA)分离术后子宫内膜及血流动力学的影响。方法选取60例中度IUA患者作为研究对象,将患者随机分为对照组与观察组,每组30例。2组均行宫腔粘连分离术(transcervical resection of adhesion,TCRA),对照组术后予以常规治疗,观察组在对照组基础上予以针刺八髎穴联合康妇消炎栓治疗,2组均治疗1个月经周期。比较2组临床总有效率、子宫内膜厚度、子宫动脉血流动力学指标、子宫内膜分型与血流深入分型以及不良反应发生情况。结果治疗后,观察组总有效率为93.33%,明显高于对照组的73.33%(P<0.05);观察组子宫内膜厚度、子宫内膜容积均高于对照组(P均<0.05);观察组子宫内膜螺旋动脉血管指数(vascular index,VI)、血流指数(flow index,FI)、血管血流指数(vascular flow index,VFI)与双侧子宫动脉阻力指数(resistance index,RI)、搏动指数(pulsation index,PI)均低于对照组(P均<0.05);观察组子宫内膜(A+B)型与血流深入Ⅱ、Ⅲ型占比均高于对照组(P均<0.05),子宫内膜C型与血流深入Ⅰ型占比均低于对照组(P均<0.05)。结论针刺八髎穴联合康妇消炎栓应用于中度宫腔粘连TCRA术后患者,可有效改善其子宫内膜结构及血流情况,缓解临床症状。

Objective To investigate the effects of acupuncture at Baliao points combined with Kangfu Xiaoyan suppository on the endometrium and hemodynamics after moderate intrauterine adhesion(IUA)separation surgery.Methods Sixty patients with moderate IUA were selected as the study subjects,and were randomly divided into the control group and the observation group,with 30 cases in each group.Both groups underwent transcervical resection of adhesion(TCRA)surgery.The control group received routine treatment after surgery,while the observation group received acupuncture at Baliao points combined with Kangfu Xiaoyan suppository on the basis of the control group.Both groups were treated for one menstrual cycle.The total clinical effective rate,endometrial thickness,uterine artery hemodynamic indicators,endometrial classification and blood flow depth classification and the incidence of adverse reactions between the two groups were compared.Results After treatment,the total effective rate of the observation group was 93.33%,which was significantly higher than the 73.33% of the control group(P<0.05).The thickness and volume of the endometrium in the observation group were higher than those in the control group(all P<0.05).The vascular index(VI),flow index(FI),vascular flow index(VFI),bilateral uterine artery resistance index(RI)and pulsation index(PI)of the observation group were all lower than those of the control group(all P<0.05).The proportion of endometrial(A+B)type and blood flow depth type Ⅱ and Ⅲ in the observation group were higher than those in the control group(all P<0.05),while the proportion of endometrial C type and blood flow depth typeⅠwere lower than those in the control group(all P<0.05).Conclusion Acupuncture at the Baliao points combined with Kangfu Xiaoyan suppository could effectively improve the endometrial structure and blood flow in patients with moderate intrauterine adhesion after TCRA surgery,and alleviate clinical symptoms.
