摘要
目的探讨外周血白细胞包括白细胞(WBC)、中性粒细胞(NEUT)、淋巴细胞(LY)、嗜酸性粒细胞(EO)、嗜碱性粒细胞(BA)及单核细胞(MO)在PsV中的表达及临床意义。方法回顾性分析2021年9月—2023年10月某医科大学第一附属医院皮肤性病科收治的105例PsV患者(实验组)的血常规中外周血白细胞指标资料,与同期在本院进行体检的98名健康体检者(对照组)的外周血白细胞资料比较,用Spearman相关分析探讨PsV患者PASI评分与WBC、NEUT、LY、EO、BA及MO的相关性,用Logistic回归分析探讨PsV的相关危险因素,用受试者工作特征(ROC)曲线探讨不同变量对PsV的诊断效能。结果与对照组比较,PsV患者NEUT及EO均升高,LY及BA降低(P<0.05);PsV患者中:WBC、LY、EO、MO与PASI评分有关(P<0.05),且与PsV患者PASI评分呈正相关;Logistic回归分析显示NEUT及EO均为PsV的危险因素,LY是PsV的保护因素。ROC曲线显示EO诊断PsV的ROC曲线面积是面积(AUC)为0.59,诊断效能最佳截断点为0.14,此时敏感度为59.0%,特异度为58.20%。结论PsV患者NEUT及EO升高,且与PASI评分呈正相关,是PsV的危险因素,EO对PsV有一定的预测价值。
Objective To investigate the expression and clinical significance of peripheral blood leukocytes,including white blood cell(WBC),neutrophil(NEUT),lymphocyte(LY),eosinophil(EO),basophil(BA),and monocyte(MO),in patients with psoriasis vulgaris(PsV).Methods A retrospective analysis was conducted on the peripheral blood leukocyte indices from the routine blood tests of 105 PsV patients(experimental group)admitted to the Dermatology and Venereology Department of the First Affiliated Hospital of a Medical University from September 2021 to October 2023.These were compared with the data from 98 healthy individuals(control group)who underwent physical examinations at the same hospital during the same period.Spearman correlation analysis was used to explore the correlation between PASI scores and WBC,NEUT,LY,EO,BA,and MO in PsV patients.Logistic regression analysis was employed to identify risk factors associated with PsV,and the diagnostic efficacy of different variables for PsV was evaluated using the receiver operating characteristic(ROC)curve.Results Compared to the control group,PsV patients exhibited increased NEUT and EO and decreased LY and BA(P<0.05).In PsV patients,WBC,LY,EO,and MO were correlated with PASI scores(P<0.05),showing a positive correlation.Logistic regression analysis indicated that NEUT and EO are risk factors for PsV,while LY is a protective factor.The ROC curve for EO in diagnosing PsV had an area under the curve(AUC)of 0.59,with the optimal diagnostic cut-off point at 0.14,yielding a sensitivity of 59.0%and a specificity of 58.20%.Conclusions In PsV patients,elevated NEUT and EO levels are positively correlated with PASI scores and are risk factors for PsV.EO has certain predictive value for PsV.
