摘要
目的探讨妊娠期高血糖(HIP)患者血清25羟基维生素D[25(OH)D]、Hcy水平与骨代谢生化指标的关系。方法选择2021年1月—2023年12月本院收治的62例HIP患者为观察组,另选择同期健康孕妇40名为对照组。检测并比较两组血清25(OH)D、Hcy水平及骨代谢生化指标[I型胶原C端肽β降解产物(β-CTX)、骨钙素(OC)、Ⅰ型胶原氨基端延长肽(PINP)、甲状旁腺素(PTH)、骨密度(BMD)值]水平,通过Spearman秩相关分析HIP患者血清25(OH)D、Hcy水平与骨代谢生化指标的相关性。结果观察组血清25-(OH)D水平低于对照组,血清Hcy水平高于对照组,差异有统计学意义(P<0.05);观察组血清PINP、PTH、OC、BMD指标低于对照组,血清β-CTX指标高于对照组,差异有统计学意义(P<0.05);HIP患者血清25(OH)D水平与PINP、PTH、OC、BMD均呈正相关,与β-CTX呈负相关,HIP患者Hcy水平与β-CTX呈正相关,与PINP、PTH、OC、BMD呈负相关,且均P<0.05。结论HIP患者血清25(OH)D水平与骨代谢生化指标联系密切,可作为该类患者病情评估的临床参考指标。
Objective To explore the relationship between serum 25-hydroxyvitamin D[25(OH)D],Hcy levels,and biochemical indicators of bone metabolism in patients with hyperglycemia in pregnancy(HIP).Methods 62 HIP patients who were admitted to our hospital from January 2021 to December 2023 were selected as the observation group,and 40 healthy pregnant women during the same period were selected as the control group.Detected and compared serum 25(OH)D,Hcy levels,and bone metabolism biochemical indicators between two groups of patients[C-terminal peptideβ-degradation products of type I collagen(β-CTX),osteocalcin(OC),N-terminal propeptide of type 1precollagen(PINP),parathyroid hormone(PTH),and bone density(BMD)].The correlation between serum 25(OH)D,Hcy levels and biochemical indicators of bone metabolism in HIP patients was analyzed using Spearman rank correlation analysis.Results The results showed that the serum 25-(OH)D level in the observation group was lower than that in the control group,and the serum Hcy level was higher than that in the control group,with statistical significance(P<0.05).The serum PINP,PTH,OC,and BMD indicators in the observation group were lower than those in the control group,and the serumβ-CTX index was higher than that of the control group,and the differences were statistically significant(P<0.05).The serum 25(OH)D level in HIP patients was positively correlated to PINP,PTH,OC and BMD;and was negatively correlated toβ-CTX.Hcy levels in HIP patients were positively correlated toβ-CTX,however,negatively correlated to PINP,PTH,OC,and BMD,all P<0.05.Conclusions The serum 25(OH)D level in HIP patients is closely related to bone metabolism biochemical indicators and can serve as a clinical reference indicator for evaluating the condition of such patients.
