某肿瘤医院2002—2017年原发小肠癌生存分析

Survival analysis of primary small intestine cancer in a cancer hospital from 2002 to 2017

目的对南通市某肿瘤医院原发小肠癌患者进行生存分析。方法收集该医院2002—2017年间新诊断的南通户籍小肠癌病例资料,随访了解患者生存状况,随访时间截至2020年12月31日。采用Kaplan-Meier法绘制生存曲线,单因素分析采用Log-rankχ^(2)检验。当生存曲线有交叉时,对交叉前后分别进行生存分析。比较不同性别、诊断年龄、户籍、肿瘤解剖部位生存率的差异。结果共纳入病例181例,男性105例,女性76例,无失访;诊断年龄60~<80岁的占58.6%。患者5年观察生存率为38.1%,平均生存期为6.11(95%CI:4.96~7.25)年。不同性别、户籍、肿瘤解剖部位的生存率差异无统计学意义(P值均>0.05);不同诊断年龄生存率差异有统计学意义(χ^(2)=11.56,P<0.01),诊断年龄35~<60岁患者生存率较高。结论小肠癌生存率较低,应加强高年龄人群小肠癌综合防治工作,早诊早治以改善预后。

Objective To investigate the survival of patients with primary small intestine cancer in a cancer hospital in Nantong City.Methods Demographic and clinical data were captured from patients with newly diagnosed small intestine cancer and household registration in Nantong City that were admitted to the hospital from 2002 to 2017,and patients′survival were followed up until December 31,2020.The survival curve was plotted using Kaplan-Meier estimates,and univariate analysis was performed using log-rank chi-square test.If the survival curves intersected,survival analyses were performed before and after the intersection.The survival rates were compared among patients with different genders,ages at diagnosis,household registrations,anatomic sites of cancer.Results A total of 181 cases with primary small intestine cancer were enrolled,including 105 men and 76 women,and there was no case lost to follow up.There were 58.6%of cases diagnosed at ages of 60 to<80 years,and the 5-year observed survival rate was 38.1%,with a mean survival period of 6.11 years[95%CI:(4.96,7.25)years].In addition,there were no gender-,household registration-or anatomic site-specific survival rates ofprimary small intestine cancer(all P values>0.05).Conclusions The survival rate of primary small intestine cancer is low.Intensified compressive management of small intestine cancer is required among residents at advanced ages,and early diagnosis and early treatment is recommended to improve the prognosis.
