医保支付方式改革下某科室费用管理策略研究

Research on Cost Management Strategy of a Certain Department under the Reform of Medical Insurance Payment

目的找出影响临床科室费用的关键因素,为科室管理策略的调整提供支持,为医保政策的顺利实施提供参考依据。方法通过对按疾病诊断相关分组(DRGs)政策实施后科室内的费用情况分析,样本非正态,用中位数和四分位间距描述,用非参数检验的方法进行检验。结果科内费用影响的重要因素是主诊医师,主诊医师对于付费标准的差值、住院天数、药品费、材料费、检查费有显著影响(P<0.05)。结论在临床科室的管理中,需加强主诊医师管理,调整科室发展策略,同时不断调整和完善医保政策,助力医疗机构的精细化管理。

Objective:To find out the key factors affecting the cost of clinical departments and provide support for the adjustment of department management strategies and a reference for the smooth implementation of medical insurance policy.Method:The cost of departments after the implementation of DRG policy was analyzed.The samples were non-normal,described by median and interquart interval,and tested with non parametric testing method.Result:The important factor of intra-department cost was the chief physician,who had significant infuence on the difference of payment standard.length of stay,drug cost,material cost and examination cost(P<0.05).Conclusion:In the management of clinical departments.it is necessary to strengthen the management of the chief physician,adjust the development strategies of the department,and constantly adjust and improve the medical insurance policy,which is helpful for the refined management of medical institutions.
