多层螺旋CT参数联合血清AFP、AFU、PIVKA-Ⅱ检测对老年原发性肝癌患者的诊断价值

Diagnostic value of multislice spiral CT parameters combined with serum AFP, AFU and PIVKA-Ⅱ in elderly patients with primary hepatic carcinoma

目的 探讨多层螺旋CT(MSCT)参数联合血清甲胎蛋白(AFP)、α-L-岩藻糖苷酶(AFU)、维生素K缺乏或拮抗剂Ⅱ诱导的蛋白质(PIVKA-Ⅱ)检测对老年原发性肝癌(PHC)患者的诊断价值。方法 选取100例老年PHC患者作为PHC组,另选取同期的100例良性肝病患者作为良性肝病组。所有受试者均进行MSCT检查,记录肝动脉灌注量(HAP)、血流量(BF)、平均通过时间(MTT)及对比剂到达时间(IRF To),检测血清AFP、AFU、PIVKA-Ⅱ水平,评估其联合检测对老年PHC的诊断效能。结果 PHC组的HAP、BF高于良性肝病组,MTT长于良性肝病组,IRF To低于良性肝病组(P<0.05)。PHC组的血清AFP、AFU、PIVKA-Ⅱ水平高于良性肝病组(P<0.05)。Ⅲ~Ⅳ期PHC患者的血清AFP、AFU、PIVKA-Ⅱ水平高于Ⅰ~Ⅱ期PHC患者(P<0.05)。MSCT参数及血清AFP、AFU、PIVKA-Ⅱ联合检测对老年PHC患者的诊断效能优于单独检测(P<0.05)。结论 MSCT参数联合血清AFP、AFU、PIVKA-Ⅱ检测对老年PHC患者的诊断效能较好。

Objective To investigate the diagnostic value of multislice spiral CT(MSCT)parameters combined with serum alphafetoprotein(AFP),alpha-L-fucosidase(AFU),protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ)in elderly patients with primary hepatic carcinoma(PHC).Methods A total of 100 elderly patients with PHC were selected as PHC group,and 100 patients with benign liver disease during the same period were selected as benign liver disease group.All subjects underwent MSCT examination,and the hepatic artery perfusion(HAP),blood flow(BF),mean transit time(MTT)and impulse residue function to(IRF To)were recorded,and the levels of serum AFP,AFU and PIVKA-Ⅱwere detected to evaluate the diagnostic efficacy of combined detection in elderly PHC.Results HAP,BF in the PHC group were higher than those in the benign liver disease group,MTT was longer than that in the benign liver disease group and IRF To was lower than that in the benign liver disease group(P<0.05).The levels of serum AFP,AFU and PIVKA-Ⅱin the PHC group were higher than those in the benign liver disease group(P<0.05).The levels of serum AFP,AFU and PIVKA-Ⅱin patients with the stageⅢ-ⅣPHC were higher than those in patients with the stageⅠ-ⅡPHC(P<0.05).Diagnostic efficacy of combined detection of MSCT parameters and serum AFP,AFU and PIVKA-Ⅱin elderly patients with PHC was better than that of single detection(P<0.05).Conclusion The diagnostic efficacy of MSCT parameters combined with serum AFP,AFU and PIVKA-Ⅱin elderly patients with PHC is better.
