中药药浴辅助间歇蓝光照射治疗新生儿高胆红素血症的效果及对患儿免疫功能和胆红素水平的影响

摘要 目的 研究中药药浴辅助间歇蓝光照射治疗新生儿高胆红素血症(NHB)的效果及对患儿免疫功能和胆红素水平的影响。方法 以2019年7月-2023年7月赣州市南康妇幼保健院接收的58例NHB患儿为研究对象,经随机数字表法分为对照组(29例)与观察组(29例),对照组采用间歇蓝光照射治疗,观察组在其基础上联合中药药浴治疗。比较两组血清胆红素水平[总胆红素(TBIL)、间接胆红素(IBIL)]、胆红素下降情况、排便频率、睡眠时长、免疫功能(外周血CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、胎粪转黄时间及黄疸消退时间。结果 两组治疗后TBIL、IBIL水平均低于治疗前,且观察组治疗后TBIL、IBIL水平低于对照组(P<0.05)。观察组治疗期间TBIL日均下降值与IBIL日均下降值均大于对照组(P<0.05)。两组治疗后排便频率、睡眠时长均大于治疗前,且观察组排便频率、睡眠时长大于对照组(P<0.05)。两组治疗后CD4^(+)、CD4^(+)/CD8^(+)高于治疗前,且观察组CD4^(+)、CD4^(+)/CD8^(+)高于对照组(P<0.05)。观察组胎粪转黄时间、黄疸消退时间短于对照组(P<0.05)。结论 中药药浴辅助间歇蓝光照射疗法可加速NHB患儿胆红素下降,增加其排便频率,延长睡眠时间,有助于改善患儿免疫功能,且可缩短其黄疸消退时间。 Objective To study the effect of traditional Chinese medicine bath assisted intermittent blue light irradiation on neonatal hyperbilirubinemia(NHB)and its effect on immune function and bilirubin level.Methods From July 2019 to July 2023,58 children with NHB received by Ganzhou Nankang Maternal and Child Health Hospital were selected as the research objects.They were divided into control group(29 children)and observation group(29 children)by random number table method.The control group was treated with intermittent blue light irradiation,and the observation group was treated with traditional Chinese medicine bath on the basis of the control group.The serum bilirubin levels[total bilirubin(TBIL),indirect bilirubin(IBIL)],bilirubin decline,defecation frequency,sleep duration,immune function(peripheral blood CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),meconium yellowing time and jaundice regression time were compared between the two groups.Results The levels of TBIL and IBIL in the two groups after treatment were lower than those before treatment,and the levels of TBIL and IBIL in the observation group after treatment were lower than those in the control group(P<0.05).The average daily decrease of TBIL and IBIL in the observation group was greater than that in the control group(P<0.05).After treatment,the defecation frequency and sleep duration of the two groups were higher than those before treatment,and the defecation frequency and sleep duration of the observation group were longer than those of the control group(P<0.05).After treatment,CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before treatment,and CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05).The time of meconium turning yellow and jaundice regression in the observation group were shorter than those in the control group(P<0.05).Conclusion Traditional Chinese medicine bath assisted intermittent blue light irradiation therapy can accelerate the decrease of bilirubin in children with NHB,increase the frequency of defecation,prolong the sleep time,help to improve the immune function of children,and shorten the time of jaundice regression.
