血管性血友病因子、肾损伤分子-1、肝细胞生长因子在急性肾损伤患者中的表达及与预后相关性

摘要 目的:分析血管性血友病因子(vWF)、肾损伤分子-1(KIM-1)、肝细胞生长因子(HGF)在急性肾损伤患者中的表达及与预后相关性。方法:选取2020年01月—2022年12月我院收治的125例急性肾损伤患者为研究对象,作为研究组,另外选取70例健康无疾病的体检者作为对照组。观察各组vWF、KIM-1、HGF水平,分析与预后的相关性。结果:与对照组相比,研究组APACHEⅡ评分、SOFA评分较高(P<0.05)。与Ⅰ期患者相比,Ⅱ期、Ⅲ期患者vWF、KIM-1、HGF表达水平逐渐升高(P<0.05)。与存活患者相比,死亡患者vWF、KIM-1、HGF表达水平较高(P<0.05)。vWF、KIM-1、HGF与预后呈正相关(P<0.05)。Logistic回归分析显示,肺部感染、APACHEⅡ评分、SOFA评分、氧合指数、Scr、BUN、eGFR、动脉血乳酸、vWF、KIM-1、HGF为影响急性肾损伤患者预后的危险因素。结论:vWF、KIM-1、HGF在急性肾损伤患者体内表达水平较高,且随着病情严重程度的增加,KIM-1、HGF水平逐渐升高,KIM-1、HGF与不良预后呈正相关,为急性肾损伤的危险因素。 Objective:To analyze the expression of vascular hemophilia factor(vWF),kidney injury molecule-1(KIM-1),and hepatocyte growth factor(HGF)in patients with acute kidney injury and their correlation with prognosis.Methods:125 patients with acute kidney injury admitted to our hospital from January 2020 to December 2022 were selected as the research group,and an additional 70 healthy and disease-free individuals were selected as the control group.Observe the levels of vWF,KIM-1,and HGF in each group and analyze their correlation with prognosis.Results:Compared with the control group,the APACHE II and SOFA scores in the study group were higher(P<0.05).Compared with stage I patients,the expression levels of vWF,KIM-1,and HGF in stageⅡandⅢpatients gradually increased(P<0.05).Compared with surviving patients,the expression levels of vWF,KIM-1,and HGF in deceased patients were higher(P<0.05).VWF,KIM-1,HGF were positively correlated with prognosis(P<0.05).Logistic regression analysis showed that pulmonary infection,APACHEⅡscore,SOFA score,oxygenation index,Scr,BUN,eGFR,arterial blood lactate,vWF,KIM-1,HGF were risk factors affecting the prognosis of patients with acute kidney injury.Conclusion:The expression levels of vWF,KIM-1,and HGF are relatively high in patients with acute kidney injury,and as the severity of the condition increases,the levels of KIM-1 and HGF gradually increase.KIM-1 and HGF are positively correlated with poor prognosis and are risk factors for acute kidney injury.
