摘要
目的探讨国产康多机器人在保留后尿道黏膜前列腺切除术中的可行性。方法选取2023年4-5月暨南大学附属第六医院收治的3例前列腺增生患者作为研究对象。平均年龄70.3岁,术前平均总前列腺特异性抗原4.984 ng/ml,平均前列腺大小52.03 cm^(3)。手术均经腹膜外途径,所有患者采用基于国产康多机器人辅助的保留后尿道黏膜前列腺摘除术治疗,术后复查彩色多普勒超声,并对患者术后疗效进行随访。结果3例患者均顺利完成手术,手术时间分别为205、260和211 min;术中失血量分别为50、50和300 ml;患者尿道黏膜破裂1例;术后均未进行膀胱冲洗,所有患者保留尿管2周,去除导尿管后可自行排尿,仅有轻度尿路刺激症状;术后无前列腺感染、继发性出血、尿失禁及附睾炎等并发症,平均随访5.67个月。结论国产康多机器人在保留后尿道黏膜前列腺切除术在治疗中等以上腺体具有安全可行、学习曲线短、患者失血量少、导尿时间短、住院时间短等优势,患者术后排尿通畅,无明显并发症。
Objective To evaluate the feasibility of the domestically produced Kangduo endoscopic surgery robot system in posterior urethra-sparing laparoscopic prostate enucleation.Methods Three patients with prostatic hyperplasia were enrolled from the Sixth Affiliated Hospital of Jinan University from April 2023 to May 2023,with a mean age of 70.3 years.The average preoperative prostate specific antigen was 4.984 ng/ml,the average prostate size was 52.03 cm^(3),and the surgery was performed via the extraperitoneal route.All patients underwent a domestically produced Kangduo endoscopic surgery robot system.Postoperative ultrasound examination was performed,and patients were followed up for postoperative outcomes.Results All 3 patients successfully completed the operation,with operative times of 205,260,and 211 min,respectively;intraoperative blood loss was 50,50,and 300 ml,respectively;the urethral mucosa was ruptured in 1 patient;no bladder irrigation was performed postoperatively,and all patients retained the urinary catheter for 2 weeks and could urinate on their own after removal of the catheter,with only mild urinary tract irritation symptoms.There were no postoperative complications such as prostate infection,secondary bleeding,urinary incontinence or epididymitis.The average follow-up time was 5.67 months.Conclusion The domestic Kangduo endoscopic surgery robot system has the advantages of safety and feasibility,short learning curve,low blood loss,short indwelling urinary catheter time,and short hospitalization time in posterior urethra-sparing laparoscopic prostate enucleation for the treatment of more than medium glands,and the patients have smooth urination with no significant complications after the operation.
