摘要
目的探讨宫颈癌患者血清诱骗受体3(DcR3)、Bmi-1、三叶因子3(TFF3)表达水平及诊断价值。方法选取2019年8月至2023年8月在该院住院的98例初诊宫颈癌患者作为宫颈癌组。另选取同期于该院就诊的74例宫颈良性病变患者和62例健康体检者分别作为良性病变组和健康对照组,收集并整理所有研究对象的临床资料。采用酶联免疫吸附试验检测血清DcR3、TFF3表达水平。采用实时荧光定量聚合酶链反应检测Bmi-1表达水平。绘制受试者工作特征(ROC)曲线评估血清Bmi-1、DcR3、TFF3单独及三者联合检测对宫颈癌的诊断价值。结果与健康对照组比较,良性病变组、宫颈癌组血清DcR3、Bmi-1、TFF3表达水平均明显升高,且宫颈癌组血清DcR3、Bmi-1、TFF3表达水平均明显高于良性病变组,差异均有统计学意义(P<0.05)。不同年龄、病理组织类型宫颈癌患者血清Bmi-1、DcR3、TFF3表达水平比较,差异均无统计学意义(P>0.05)。不同国际妇产科联盟分期、组织分化程度、淋巴结转移宫颈癌患者血清Bmi-1、DcR3、TFF3表达水平比较,差异均有统计学意义(P<0.05)。ROC曲线分析结果显示,血清DcR3、Bmi-1、TFF3水平单独及三者联合检测诊断宫颈癌的曲线下面积(AUC)分别为0.829、0.851、0.841、0.918,三者联合检测优于各自单独检测的AUC(Z_(三者联合-DcR3)=2.641,P=0.008,Z_(三者联合-Bmi-1)=2.201,P=0.028,Z_(三者联合-TFF3)=2.971,P=0.003)。结论宫颈癌患者血清中DcR3、Bmi-1、TFF3水平明显升高,三者联合检测诊断宫颈癌的临床价值较高。
Objective To investigate the expression levels of decoy receptor 3(DcR3),Bmi-1 and trefoil factor 3(TFF3)in serum of patients with cervical cancer and their diagnostic value.Methods A total of 98 patients with newly diagnosed cervical cancer who were hospitalized in the hospital from August 2019 to August 2023 were selected as the cervical cancer group.In addition,74 patients with benign cervical lesions and 62 healthy people were selected as the benign lesion group and the healthy control group respectively.The clinical data of all the research objects were collected and sorted out.Enzyme-linked immunosorbent assay was used to detect the expression levels of serum DcR3 and TFF3.The expression level of Bmi-1 was detected by real-time fluorescence quantitative polymerase chain reaction.The receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of serum Bmi-1,DcR3,TFF3 alone and combined detection of the three for cervical cancer.Results Compared with the healthy control group,the levels of serum DcR3,Bmi-1 and TFF3 in the benign lesion group and the cervical cancer group were significantly increased,and the expression levels of serum DcR3,Bmi-1 and TFF3 in the cervical cancer group were significantly higher than those in the benign lesion group,and the differences were statistically significant(P<0.05).There was no significant difference in the expression levels of serum Bmi-1,DcR3 and TFF3 in cervical cancer patients with different ages and pathological types(P>0.05).There were statistically significant differences in the expression levels of serum Bmi-1,DcR3 and TFF3 in cervical cancer patients with different International Federation of Gynecology and Obstetrics stages,tissue differentiation and lymph node metastasis(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of serum DcR3,Bmi-1,TFF3 levels alone and in combined detection in the diagnosis of cervical cancer were 0.829,0.851,0.841,0.918 respectively,the combined detection of the three levels was better than the AUC of each single detection(Z_(combined-DcR3)=2.641,P=0.008,Z_(three combination-Bmi-1)=2.201,P=0.028,Z_(three combination-TFF3)=2.971,P=0.003).Conclusion The levels of DcR3,Bmi-1 and TFF3 in serum of patients with cervical cancer are significantly increased,and the combination of the three has high clinical value in the diagnosis of cervical cancer.
