化痰止咳合剂联合西药治疗社区获得性肺炎疗效观察及对肺功能、免疫功能的影响

Observation on Curative Effect of Combination Use of Expectorant Cough Mixture and Western Medicine on Community Acquired Pneumonia and Its Effect on Lung Function and Immune Function

目的:观察化痰止咳合剂联合西药治疗社区获得性肺炎(CAP)的疗效及对患者肺功能、免疫功能的影响。方法:选取96例CAP患者,按随机数字表法分为对照组及观察组各48例。对照组给予西药对症治疗,观察组在对照组基础上给予化痰止咳合剂治疗。比较2组临床疗效及症状缓解时间,比较2组治疗前后炎症指标值[降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)、超敏C-反应蛋白(hs-CRP)]、肺功能指标值[用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC]及免疫功能(T淋巴细胞亚群CD4^(+)、CD8^(+))水平的变化。结果:观察组临床疗效总有效率为95.83%,对照组83.33%,2组比较,差异有统计学意义(P<0.05)。观察组退热时间、咳嗽消失时间、咳痰消失时间、肺啰音消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,2组hs-CRP、PCT、TNF-α水平均较治疗前下降(P<0.05),观察组上述3项指标水平均低于对照组(P<0.05)。治疗后,2组FEV_(1)、FVC、FEV_(1)/FVC水平均较治疗前升高(P<0.05),观察组上述3项指标水平均高于对照组(P<0.05)。治疗后,2组CD4^(+)、CD4^(+)/CD8^(+)水平均较治疗前升高(P<0.05),CD8^(+)水平均较治疗前下降(P<0.05);观察组CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组(P<0.05),CD8^(+)水平低于对照组(P<0.05)。结论:化痰止咳合剂联合西药治疗CAP效果显著,可缓解症状,对减轻炎症反应、改善肺功能及提升机体免疫力有积极作用。

Objective:To observe the curative effect of combination use of Expectorant Cough Mixture and western medicine on community acquired pneumonia(CAP)and its effects on the lung function and immune function of patients.Methods:A total of 96 CAP patients were selected and divided into the control group and the observation group according to the random number table method,with 48 cases in each group.The control group was given symptomatic treatment with western medicine,and the observation group was treated with Expectorant Cough Mixture based on the treatment of the control group.The changes in clinical effects,relief time of symptoms,inflammatory indexes[procalcitonin(PCT),tumor necrosis factor-α(TNF-α)and hypersensitive C-reactive protein(hs-CRP)],pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in the first second(FEV_(1)),and FEV_(1)/FVC]and immune function(levels of T lymphocyte subsets CD4^(+)and CD8^(+))were compared between the two groups.Results:The total effective rate was 95.83%in the observation group and 83.33%in the control group,the difference being significant(P<0.05).The disappearance time of fever,cough,expectoration and lung rales in the observation group was respectively earlier than that in the control group,differences being significant(P<0.05).After treatment,the levels of hs-CRP,PCT and TNF-αin the two groups were decreased when compared with those before treatment(P<0.05),and the above three indexes in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of FEV_(1),FVC and FEV_(1)/FVC in the two groups were increased when compared with those before treatment(P<0.05),and the above three indexes in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)in the two groups were elevated when compared with those before treatment(P<0.05),and the levels of CD8^(+)in the two groups were reduced when compared with those before treatment(P<0.05);the levels of CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05),and the levels of CD8^(+)was lower than that in the control group(P<0.05).Conclusion:The combination use of Expectorant Cough Mixture and western medicine has a significant curative effect on CAP,which can mitigate the inflammatory responses,improve the lung function and immune function of the body.
