3.0T高分辨磁共振成像在后循环缺血性脑卒中患者斑块评估中的应用

摘要 目的 探讨3.0T高分辨磁共振成像(HR-MRI)对后循环缺血性脑卒中患者斑块的评估作用。方法回顾性分析河北北方学院附属第一医院2021年5月至2023年4月收治的83例后循环缺血症状患者临床资料,所有患者均进行3.0T HR-MRI检查,根据患者是否为后循环缺血性脑卒中将其分为后循环缺血性脑卒中组(n=43)和非脑卒中组(n=40),通过MRI血管成像、高分辨T_(1)加权成像(T_(1)WI)等观察和测量两组患者斑块血管狭窄情况以及斑块分布情况和特征,对比两组后循环动脉狭窄程度、斑块分布情况、斑块负荷、斑块信号、斑块形态、斑块强化、斑块厚度及斑块体积等情况。结果脑卒中组后循环动脉斑块分布与非脑卒中组比较差异均无统计学意义(P>0.05),动脉狭窄程度高于非脑卒中组(P<0.05)。脑卒中组斑块负荷、斑块厚度和斑块体积大于非脑卒中组(P<0.05),斑块强化比例和斑块内高信号比例高于非脑卒中组(P<0.05),斑块形态与非脑卒中组比较差异均无统计学意义(P>0.05)。脑卒中组斑块最厚点位于左/右侧壁比例高于非脑卒中组(χ^(2)=5.846,P<0.05),斑块最厚点位于腹侧壁、背侧壁比例与非脑卒中组比较差异均无统计学意义(P>0.05)。结论后循环缺血性脑卒中患者动脉狭窄程度更显著,动脉粥样硬化斑块负荷、斑块强化和斑块内高信号也相对常见,3.0T HR-MRI有利于评估后循环缺血性脑卒中患者斑块特征。 Objective To investigate the effect of 3.0T high resolution magnetic resonance imaging(HR-MRI) on plaque evaluation in patients with posterior circulation ischemic stroke.Methods Clinical data of83 patients with post-circulation ischemia admitted to the First Affiliated Hospital of Hebei North University from May 2021 to April 2023 were retrospectively analyzed.All patients underwent 3.0T HR-MRI examination.According to whether the patients had post-circulation ischemic stroke,they were divided into post-circulation ischemic stroke group(n=43) and non-stroke group(n=40).MRI angiography and high-resolution Ti-weighted imaging(T_1WI) were used to observe and measure the plaque vascular stenosis,plaque distribution and characteristics of the two groups.The degree of arterial stenosis,plaque distribution,plaque load,plaque signal,plaque morphology,plaque enhancement,plaque thickness and plaque volume were compared between the two groups.Results There was no significant difference in the distribution of posterior arterial plaque between the stroke group and the non-stroke group(P>0.05),and the degree of arterial stenosis was higher than that of the non-stroke group(P<0.05).The plaque load,plaque thickness and plaque volume in the stroke group were higher than those in the non-stroke group(P<0.05),the proportion of plaque enhancement and the proportion of high signal in the plaque were higher than those in the nonstroke group(P<0.05),and there was no statistical significance in plaque morphology between the two groups(P>0.05).The ratio of the thickest point of plaque on the left and right lateral wall in the stroke group was higher than that in the non-stroke group(χ^(2)=5.846,P<0.05),and the ratio of the thickest point of plaque on the venstral wall and the dorsal wall had no statistical significance between the two groups(P>0.05).Conclusion The degree of a rterial stenosis is more significant in patients with postcirculation ischemic stroke,and atherosclerotic plaque load,plaque enhancement and intracavicular high signal are relatively common.3.0T HR-MRI is beneficial for evaluating plaque characteristics in patients with post-circulation ischemic stro ke.
